What you'll be working: Area: Live in and around North Yorkshire/ West Yorks area Office is based in Darlington Preferable case management experience but not essential Salary: 45k to 50k Depending on experience Working Hybrid - Mostly Homebased with 1 to 2 days in office per month. The Role As an experienced clinician, you'll use your clinical skills and manage your own caseload of clients. The organisation supports clients in the North West and Yorkshire who have an acquired a brain injury, spinal injury or life-changing physical disabilities due to an accident or a birth injury. When a referral is received, you'll be responsible for completing a through and detailed assessment of their needs. They take a holistic approach so this will include their care, emotional and social needs. You'll implement a rehabilitation package and continually review and assess it, to ensure that it continues to meet the client's needs. Most clients are involved in a medico-legal process and you'll support their case through accurate report writing and continual re-assessment. You'll be able to put into place bespoke equipment, vehicles, holiday's etc. that will benefit the clients health and emotional needs. You'll work within a Multi-disciplinary team consisting of nurses, OT's, Social Workers and Physio's and be able to share knowledge and experiences with others in the office. What you'll be doing: As Case Managers, we continuously work with our clients and their families to assess rehabilitation needs, commission services and lead multi-disciplinary teams, providing rehabilitation and support in the community to assist clients to reach their optimum potential. What you'll have: We are seeking dynamic, passionate individuals with the following: Professionally qualified as a Social Worker, Nurse, Occupational Therapist, Physiotherapist, Speech and Language Therapist, or Psychologist. Case management experience preferred but not essential.
