Provides a complete physical assessment and history of current and previous illness(es). The expectation is this will happen prior to start of care, or within 24 hours if care begins on a weekend. Regularly re-evaluates patient nursing needs. At least each 60 days, or prn change in status. Initiates the plan of care and makes necessary revisions as patient status and needs change. Uses health assessment data to determine nursing diagnosis. Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician. Provides direct patient care as defined in the State Nurse Practice Act. Counsels the patient and family in meeting nursing and related needs. Provides health care instructions to the patient as appropriate per assessment and plan of care. Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient. Acts as Case Manager and assumes responsibility to coordinate patient care for assigned caseload. Will be present on campus on their assigned weekends as case load dictates, up to 16 hours, to provide dressing changes, pill box changes, new case assessments, and routine assessments as scheduled. Weekends off will be coordinated and covered by the two weekend nurses, unless prior arrangements have been made with Home Care Administrator. RNs will attend quarterly Home Care Mandatory Meetings. COMMUNICATION Prepares clinical notes and updates the primary physician when necessary and at least every 60 days. Communicates with the physician regarding the patient’s needs and reports any changes in the patient’s condition; obtains/receives physician’s orders as required. Communicates with community health related persons to coordinate the care plan. Communicates with Scheduler any changes in case hours, patient acuity, or change in care plan Communicates with staff via telephony any change in resident care plan Communicates with Home Care Administrator at least weekly with any changes in case load. Communicates with RN Case Manager to give report, explain cases from weekend, and schedule assessments as appropriate. ADDITIONAL DUTIES Participates in on-call duties as defined by the on-call policy. To include answering the RN on call phone Friday 5pm until Monday 8am. Will provide written or electronic communication to Home Care Administrator of any and all on call activity daily. Ensures that arrangements for equipment and other necessary items and services are available. Instructs, supervises and evaluates home health aide care provided, including supervisory visits, skills fairs, and new employee orientation. Case Manager will provide written feedback to be used for performance evaluations, and be available to perform those performance reviews as delegated by Home Care Administrator. Participate in campus wide Flu Clinic, as able Administer TB shots for Home Care staff as directed by H.R. Order medical supplies for Home Care office as needed. EDUCATION and/or EXPERIENCE: Graduate of an accredited school of nursing. One (1) to two (2) years of recent experience in a healthcare setting. Current licensure in state, CPR certification and valid driver’s license. Bachelor’s degree, with one (1) year of home health care experience preferred. Responsible for supervising home health aides. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. Prolonged or considerable walking or standing. Able to lift, position or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling or crouching. Visual acuity and hearing to perform required nursing skills. Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements
