Main duties of the job
About the role:
* Joining a team of one pharmacist and two pharmacy technicians.
* Main priority of the role will be to support with medicines-related activity such as completing medication reviews, SMRs, carrying out QOF work, medicines optimisation, CQC audits, and Eclipse.
* Supported by a PCN Manager, Clinical Director, Prescribing Lead GP Partner, and wider clinical workforce.

We are operating out of four sites: The Mounts Medical Centre, Favell Plus Surgery, Mayfield Surgery, and Maple Access Partnership GP surgery. We have approximately 54,000 patients. Job responsibilities
The post requires autonomous working to provide pharmaceutical care to our patients. You will have daily appointments to offer medication reviews and medication advice as well as minor alignments and other contractual obligations including QOF and IIF schemes.
* Completing medication reconciliations and reviews for patients SMR both in person and over the telephone.
* Complete SMR.
* Medicine optimisations and medicine prescriptions.
* Carry out benzodiazepine reviews.
* Prescribing medication.
* Must be able to offer sound and professional advice in relation to clinical management issues.
* Maintain and improve quality and patient safety.
* Provide pharmaceutical input for clinical governance meetings.
Person Specification
Qualifications
* Masters Degree in Pharmacy or equivalent.
* Registered with GPhC.
* Specialist knowledge through a Postgraduate Diploma & Qualification.
* Evidence of recent CPD.
* Independent Prescriber status or a commitment to undertake the course.
* Must hold a UK driver's licence and have access to a car.
* At least 2 years post-registration experience in a hospital, community, or general practice setting.
* Experience of undertaking medication or medicine use reviews, and patient counselling.
* Experience of working with clinicians and multidisciplinary/multi-agency work.
* Experience in undertaking clinical audit.
