This post is an exciting opportunity for an experienced and competent Therapist who has advanced discharge planning, assessment knowledge and skills providing support with Home First within the Integrated placement of care team (IPOCH). The post holder has decision-making autonomy and is responsible and accountable for the development, education, and supervision of others. The post holder will work collaboratively and cooperatively with others to meet the needs of patients and their families and take an active part in facilitating safe and timely discharge from hospital, Pathway 2 Discharge to Assess Beds, and ongoing assessments within community settings in line with National Hospital Discharge Guidance.
Main duties of the job
We are looking for an enthusiastic, dynamic and motivated Occupational Therapist to join our friendly and supportive IPOCH Team to drive clinical excellence and service development.
The post holder will support the triaging of patients who are ready for discharge adopting a Proportionate Care and Home First Approach, supporting with Complex Home Visits alongside the wider Multidisciplinary team. You will be required to continuously review patients listed for the Pathway 2 beds to determine suitability and update plans as required. The role will also include attending MDT meetings within the hospital setting to promote timely discharges and promote the Home First Approach. You will take a Leadership Role within the IPOCH team supervising members of the team supporting with Motiv8s, 1:1s, sickness and HR as required. The IPOCH team support the ongoing process of Continuous Improvement, and you will be required to take an active role in this. The post holder will be expected to work occasional weekends and Bank Holidays.
The IPOCH team is based at Leighton Hospital seeing patients within the hospital, Pathway 2 beds within care homes and supporting Pathway 1 patients within their own home in the community.
You are required to be a car driver and have access to your own vehicle.

Essential
* HCPC (Health and Care Professional Council) registration
* Relevant professional degree/diploma/MSc (Master of Science) pre-registration.
* Clinical Educator Training

* Post registration Masters in relevant area.
* Advanced Communication skills qualification.
* Membership of a specialist interest group relevant to the specialty.
