This is an exciting opportunity to join Swanshurst IMT. We currently have 1.0 WTE posts available. The team are based at Moseley Hall Hospital covering the local community. It has a dynamic and innovative approach to Nursing care.
We are looking for highly motivated applicants who are interested in meeting the challenge of working in the community with patients in their own homes. You will be working with a variety of disciplines within the multidisciplinary team to achieve the best outcomes for patients to ensure their independence and well-being.
Main duties of the job
1. Caring for patients within their own homes.
2. Delivering clinical care within the community.
3. Working as part of the wider MDT ensuring holistic assessment and care to meet individuals' needs.

Job responsibilities
The service operates between the hours of 8 am to 6 pm over 7 days per week and the working pattern for the successful candidate will include weekend working and bank holiday periods. We are working towards an 8-8 shift pattern which will be introduced within the next 12 months.
This post requires the successful candidates to be highly mobile so requires a full driving license and car available for work.
