The shift pattern for this position is 9am to 5pm, Monday to Friday.
The Discharge Coordinator post ensures safe, efficient and effective successful discharges for Adult Mental Health patients in hospital. The post holder will work in close partnership with members of the multidisciplinary teams to ensure robust and streamlined discharges, preventing any unnecessary delay to discharge, identifying early barriers to discharge planning and facilitating complex case discussions. The successful candidate will support the strategic planning and delivery of this service to reflect NHS Greater Glasgow and Clyde values and beliefs, policy, Nursing and Health Strategies and Governmental Targets. Ensuring that the service is delivering upon the priorities within Mental Health Strategy.
The successful candidate must be a first level Registered Mental Health Nurse with relevant knowledge training and experience to make informed decisions based on each patient’s individual situational needs. They require to be educated to Degree level or have equivalent relevant experience, with at least 2 years-experience at Band 6 level and have the ability to evidence continuing professional development.
