This is an exciting opportunity for a registered practitioner to join the community division as a Clinical Quality Lead (CQL).
The CQL focuses on the provision of high-quality care having a visible professional leadership style with a clear remit of setting/maintaining standards of clinical excellence, improving outcomes and experiences for patients. You will have a strong clinical knowledge base & expertise in the nursing care of frail/medical patients within the hospital, rehab & community setting currently working in the NHS at Band 7 or above.
You will be the Clinical Quality Lead (Matron) for our Frailty SDEC, Community Assessment Unit (CAU), Jean Heyes Reablement Unit and community services that are involved in the discharge & flow of patients across our services, using your strong leadership skills within this challenging and exciting role.
You must have excellent communication & influencing skills to develop & maintain effective relationships/networks to initiate & support change to drive new ways of working.
The role is crucial to enable safe care, safe discharge & ultimately admissions avoidance. We are looking for a motivated, inspirational professional who demonstrates excellence in practice through innovation to drive quality & safe patient outcomes in high-pressure areas.
This is a permanent full-time post (37.5 hours) working primarily Monday to Friday with on-call. There is an expectation that you will join the Duty Matron rota for the trust.
Main duties of the job
* Provide clinical and professional leadership and advice within the Division specific to our bed-based services and wider discharge and flow Teams, to ensure the safe delivery of the highest quality of patient care and service delivery.
* Be available to patients, their families and staff to provide assistance, advice, and support and to address problems and concerns as required.
* Champion that day-to-day quality care is delivered with compassion to all service users.
* Oversee and monitor the quality, timeliness and development of nursing care in accordance with relevant professional standards.
* Support the environments in which patient care and treatment takes place is safe and clean, whilst ensuring staff practice in accordance with statutory and corporate policies, including those specific to patient safety.
* Be a motivated professional who will innovate and develop team members.
* Inspire colleagues by positive role modelling, creating visions and exploring long-term ideas to improve clinical services whilst working in a climate of constant change.
* Contribute to the strategic direction of Community Division in conjunction with the Divisional Director of Nursing (DDoN) and their Deputy.

Detailed job description and main responsibilities
* Ensure self and others practice in accordance with the Nursing and Midwifery Council and HCPC standards of conduct.
* Act as an ambassador for the professional staff, ensuring self and others practice to the highest professional standards.
* Ensure all staff have clear and agreed performance objectives that support the delivery of the Division and the Trust values (mission statement).
* Participate in skill mix review, workforce redesign and recruitment and retention strategies to develop and modernise the workforce in conjunction with the Divisional Director of Nursing & AHP and Operational Managers.
* Ensure that staffing levels are appropriate to patient need by working with Unit Managers and Team Leaders to assess patient caseloads and adjust duty rotas accordingly.
* Collaborate with the Infection Prevention and Control team to reduce healthcare-related infections in accordance with national and local guidelines.
* Ensure that staff are aware of their responsibility for Safeguarding including compliance with Trust policies and safeguarding supervision where necessary.
* Undertake monthly quality reviews of documentation from IT systems and pathway/paper documents to provide assurance of the quality and service delivered to patients.
* Responsible for providing accurate and timely records on patient care and performance using paper and IT-based systems.
* Development of audits and analysis of working practices and trends to support improvements in performance in relation to quality and patient experience.
* Ensure patient confidentiality is maintained.
* Ensure effective and appropriate supervision processes are in place, providing professional and clinical supervision as required.
* Maintain a consistent visible presence within clinical areas.
* Participate in the Duty Matron role at WWL.
Planning and Organisational Duties
* Work closely with Infection, Prevention and Control to ensure all IPC policies and procedures are adhered to by monitoring and challenging poor practice.
* Implement effective risk assessment and management within own Clinical Directorate working closely with the quality and governance team.
* Accountable for the implementation of systems for monitoring the quality and safety of patient care, including the delivery of harm-free care, and experiences.
* Support the teams throughout inspections and local accreditation schemes including the development and monitoring of improvement plans.
* Accountable for ensuring the appropriate and effective development, delivery and monitoring of patient care and service across their clinical areas.
* Monitor the quality and timeliness of nursing care and service provision in accordance with relevant professional and local standards.
* Where possible, take all reasonable action to address problems, concerns and complaints as near to 'source' as possible, but also understand when to escalate.
* Support quality, governance and operational team leaders to ensure the most appropriate distribution of staffing resources.
* Work alongside the Professional Practice Team to lead, develop and maintain the in-house education/training for all staff.
* Lead on embedding action plans/learning arising from adverse events, incidents, never events, complaints in close liaison with the governance team.
* Participate in clinical audit and promote/identify research activity/opportunities that evaluate information that support clinical decision making.
* Work clinically at least 7.5 hours per month within own clinical areas of responsibility.
* Work in partnership with Operational Managers, Unit Managers, Team Leaders, Corporate teams, and wider organisational teams in preparation for and assessment of compliance with CQC fundamental standards.

Communications and Key Working Relationships
* Use information obtained from patients, families, and staff to make positive changes to the patient experience and care where appropriate or necessary.
* Investigate clinical incidents and complaints/concerns as required and ensure that effective risk management and incident investigation systems are in place.
* Take every reasonable effort to ensure that issues, concerns, and problems are managed efficiently and effectively.
* Provide professional and clinical supervision as required demonstrating advanced clinical knowledge, judgement and decision making.
* Ensure the inclusion of service users in service redesign.
Responsibility for Finance
* Support the Divisional Management Team in the procurement and commissioning of services, products, and equipment.
* Participate in regular review of establishments and skill mix ensuring appropriate levels are maintained and secured in line with service development.
* Ensure the efficient use of temporary and agency staff, monitoring quality and expenditure, authorising payments.
* Actively support the Division to identify efficiencies that will release time and/or reduce spend across clinical and non-clinical areas.
Responsibility for Human Resources
* Support the informal and formal appraisal of staff and for ensuring that they are trained, supported, and developed accordingly.
* Contribute to workforce planning for the Community Division by identifying appropriate succession planning.
* Ensure effective recruitment and retention initiatives are implemented in conjunction with senior staff.
* Support regular performance review and development plans for all staff within their area of responsibility.
* Promote a culture that values staff, recognising and acknowledging achievement whilst positively addressing poor performance.
* Ensure that staff complete the required mandatory training for their job roles.

Essential criteria
* Registered in the relevant professional Nursing/AHP fields
* Educated to Master's level or equivalent experience
* Training in leadership and management skills or equivalent experience
