Job summary The Pembrokeshire Memory Assessment Service Team are looking to recruit a Specialist Memory Nurse on a permanent basis. You will also be supported to develop your own clinical competency profile and to cultivate specialist areas of practice. The post holder will be responsible for carrying out initial assessments (including standardised cognitive tests), and participate in formulating diagnoses and care plans with the support of the MDT, communicating with clients and their carers, discussing treatment options, and signposting people to relevant support services. The Memory Service staff are expected to provide evidence based interventions and achieve positive outcomes for older adults and their carers these will be evidenced by performance indicators including activity data, targets and quality measures. Main duties of the job The post will have a key role in providing time limited support and interventions to people with dementia, their carers and relatives in both clinic and home-based settings and the post holder will liaise with colleagues within primary care, community health and social care services, voluntary organisations and the wider mental health of older adult service as appropriate. The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
