Job summary
The DCHS Diabetes Education service is for people diagnosed with type 2 diabetes living in Derbyshire and Derby City. Through our education programmes we provide support to help patients manage their diabetes.
Recently awarded X-PERT Health Organisation of the Year, we are looking to appoint a highly motivated, enthusiastic, efficient and friendly person to be part of the Administration Team.
You will need to have:
* A good standard of general education which includes English language GCSE, Grade C or above
* Experience of working in an office environment, including answering a busy telephone system, phoning patients/members of the public
* IT literacy skills
* Excellent communication and interpersonal skills
* Excellent customer care and communication skills are essential as well as a friendly and caring personality.
* You should also be able to stay calm under pressure.
* You must have good keyboard and PC skills, with demonstrable experience of using Microsoft Office, e.g. Word, Excel and Outlook
The post holder will need to be able to work from an office base and from home.
Main duties of the job
* To phone/contact patients referred into the Diabetes Education Service to explain about the diabetes education programmes and options available so that the patient can make an informed choice on the most suitable programme for their individual needs.
* To work as part of the admin team, plan and organise the on-going delivery of the Type 2 Diabetes Structured Education Programme.
* To work as part of and support the admin team with general admin tasks and duties.
* Support the Diabetes Education Co-ordinator in supporting educators to match service delivery needs.
* To provide patients basic information about the content of the diabetes education programme.
* To send out all relevant course documentation to patients who have booked onto the programme.
* To liaise with GP practices where referral forms are incomplete.

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

You will work in a friendly team with 4 other administration staff, the Programme Coordinator, Diabetes Educators and Operational Lead.

KEY DUTIES TASKS AND RESPONSIBILITIES
Patient Care
* To phone/contact patients referred into the diabetes education service to explain about the diabetes education programme and options available so that the patient can make an informed choice on the most suitable programme for their individual needs. This discussion should be patient-centred and underpinned by a quality conversations approach.
* To provide patients basic information about the content of the diabetes education programme.
* To send out all relevant course documentation to patients who have booked onto the programme.
* To liaise with GP practices where referral forms are incomplete.
* To communicate sensitively and effectively with patients and carers, in a helpful manner, when receiving referrals and responding to enquiries/concerns.
* Need to be able to deal with people with communication difficulties (e.g. hard of hearing).
* Respond to patients needs according to DCHS Policies and Procedures and in line with Caldecott Guidance and data protection regulations.
Strategy and Service Development
* To work as part of the admin team, plan and organise the on-going delivery of the Type 2 Diabetes Structured Education Programme.
* To work as part of and support the admin team with general admin tasks and duties.
* Support the Diabetes Education Coordinator in supporting educators to match service delivery needs.
Financial Resources/Management
* Ensuring the resources of the service are stored and maintained in an appropriate and secure manner.
* Support the service coordinator with the ordering of goods and services associated with the programme.
* Maintain stock of patient literature, ensuring availability of information packs for use in education sessions.
* Maintain stock of relevant forms and stationary items required to ensure the smooth running of the service.
* Responsible for ensuring essential equipment available to ensure smooth running of sessions e.g. client booklets, educator toolkits.
Information Resources/Information Systems
* To input patient data, course attendance and patient outcomes onto SystmOne and other relevant data systems.
* To check incoming emails daily via the service email and respond promptly to requests and queries.
Autonomy/Scope within Role
Although the post holder will be supervised and supported by other members of the team they will be expected to work on their own and take responsibility for the completion of tasks set.
Physical
Combination of sitting/walking. Sitting at desk using computer and inputting data. Answering telephone, making and forwarding calls and taking messages. May be required to move resource materials and equipment used in delivering courses.
Emotional
Need to empathise with members of the public/patients who may be distressed or have challenging issues.
Mental
Frequent requirement for concentration where work pattern is unpredictable. Busy office environment. Unpredictable work patterns require concentration. Concentration required when using computer and database.
Working Environment
The office can be busy and noisy and there are constant interruptions. Frequent use of VDU equipment.
