To provide efficient and effective support to the EOC Clinical Portfolio Team in the delivery of the CAD Markers Policy, and Management of Patients with Defined Individual Needs Policy.

The successful candidate will be able to work on an agile basis (3 days working from home with onsite working 2 days per week). Occasional regional travel is also required as necessary and operationally required.
· Toliaise with external agencies including other NHS Organisations, Police Forces, Local Authorities, ICBs, providing information to the agencies on behalf of the EOC Clinical Portfolio Team, including CAD Markers, Complex Case and Frequent Callers.
Action and log on DATIX all information in relation to CAD Markers, Complex Case and Frequent Caller Caseload. Manage and respond with appropriate documentation and reports for CAD Markers, Complex Case and Frequent Caller Case Management, collate information and provide initial analysis for Complex Case and Frequent Caller Leads.
· To complete general office duties (photocopying, scanning, etc) to support the EOC Clinical Portfolio Team Frequent Caller and Complex Case Leads in their work.

· Draft information and chronologies for Case Management Systems within the EOC Clinical Team.
Ensure that the correct information on Case Management is current and uploaded to the correct Trust system with a high level of accuracy.
To complete general office duties (photocopying, scanning, etc) to support the EOC Clinical Portfolio Team Frequent Caller and Complex Case Leads in their work.
To facilitate and organise meetings, attend meetings and produce reports/notes, as requested.
To maintain electronic libraries of information and resources for staff and users, including the Trust website and intranet.
· To plan and support EOC Clinical Portfolio Projects in collaboration with other Team Members.
· To collate statistical information relating to the relevant caseload and distribute to relevant personnel for promotion of management of current caseload in a timely and accurate manner.
· Collate information for Clinicians within the Team to process and manage relevant caseload.
· To manage relevant databases within the Team, as designated by their portfolio.
· To utilise Trust and external databases to complete Requests for Information and ensure that patient information held on Trust systems is accurate.
· To maintain all necessary administrative records: timesheets, & annual leave requests
Maintain an electronic filing system of Requests for Information (RFIs), reports, procedures, guidance, and information relating to the work of the EOC Clinical Portfolio Team, and provide support for multiagency projects/workstreams.
To complete Audits for Frequent Caller and Complex Case Information to ensure compliance with EEAST Policies. Collate information on monthly audit results, analyse, and publish and distribute to personnel appropriately. Feedback to all staff involved in this process.
· Maintain and amend audit process as appropriate to the needs of the EOC Clinical Portfolio Team.
· Support internal and external audits in information gathering. Facilitate information gathering for relevant dashboards internally.
· Collate/obtain actions taken from external agencies by email, letter, and telephone, regarding RFIs and Case Management, and all other cases, and appropriately log and communicate with all involved staff.
· Identify and action incidents to be reported on the DATIX System.
· Liaise with Senior Managers on individual matters relating to case management/CAD Markers providing them with the necessary information to be able to manage and review relevant queries.
· Provide administrative support to the Complex Case Lead and Frequent Caller Lead by managing their diary, undertake other administrative support.
To monitor and amend data on the Trust web/intranet page in relation to the EOC Clinical Portfolio, ensuring recent documentation is posted and old documentation removed as required.
· To undertake work as directed within the Team to support current business needs that may be different and varied.
· Manage patient records in line with current policy and NHS Record Management Code of Practice 2021, or as defined by the Complex Case/Frequent Caller Leads.
Support process management of highly sensitive data adhering to relevant IG Policy and Legislation.
· To support the management of a portfolio within the EOC Clinical Portfolio Team as defined by the Complex Case/Frequent Caller Lead.
