• The post holder will be responsible for providing excellent customer service and contributing proactive information to support the delivery of the Virtual Ward Service.
• Thepostholderwillworkcloselywithallrelevantclinicalareastosupportany Trust and ICS performance indicators/targets related to Hospital at home or the Frailty Virtual Ward Service.

Toensureallphonecallsaredealtwithinaprofessionalandtimelymanner adhering to the local targets set by the department and Trust.
To work with clinical teams on a day-to-day basis to ensure flow, coordination and administrative aspects of the service.
TosupporttheclinicalandoperationalteamstomaintainStandardOperating Procedures (SOPs) for the Virtual Ward.
To ensure all patients and visitors are treated with dignity, respect, and empathy at all times.
Todealwithpatientsinapoliteandrespectfulmannercomplyingwiththeir requests where possible.
Tosupport theTrustindeliveringNHStargetsandassist withmonitoringvirtual ward performance on a biweekly basis.
Torespondinatimelyfashiontoalladministrativedutieswithintheteam.
TosupporttheimplementationofallnewITandElectronicPatientRecordsystems.
Assist and be pro-active in all modernization projects that concern the team andimprove patient pathways.
Tobemulti-skilledinyourworkingroleandabletoassistinallareasofthedepartment as and when required.
To ensure strict confidentiality is always maintained in respect of correspondence received in the department and in accordance with Trust policy.
