1. To manage a caseload of participants who have experienced common mental, physical and social health problems or long-term conditions. 2. To carry out vocational assessments and develop Strive to Work and Thrive to Work plans with WorkWell participants. 3. Ensuring that participants are eligible to attend the service. 4. To provide an evidence based, person-centred and holistic initial assessment of barriers. 5. To provide a light-touch holistic support for health-related barriers to employment. 6. To provide a pathway into local services to tackle specific need and ensure referral pathways into and onwards from Work Well services to ensure maximum opportunities for people to engage with appropriate services. 7. To maintain accurate case notes, action plans and WorkWell participant paperwork and upload these to the management information system within the specified timeframe. 8. To provide information, advice, guidance and support to individuals about the pathways of opportunities available to them to gain, return to and sustain work. 9. To support individuals to gain accurate benefits advice, complete paid job applications, compile CVs and to prepare for interviews. 10. To liaise with and arrange meetings with health professionals, partner agencies and/or employers concerning individual needs to increase opportunities for positive outcomes for WorkWell participants. 11. To deliver training and workshops for participants on job applications, CV writing and interview skills to improve their chances of moving into employment. 12. To take part in informal training with colleagues from external organisations to help raise awareness and improve knowledge of potential barriers to employment. 13. To monitor progress and outcomes of participants supported by WorkWell, ensuring that stringent monitoring deadlines are adhered to, and KPI evidence is collected accurately. 14. To work flexible hours as required to meet the needs of the service. Alongside these main duties you will be expected to carry out any other duties that are reasonably requested of you.
