The post holder will have a specific focus on improving all aspects of discharge, supporting the wards/departments through strong clinical leadership, service improvement and development, and improving the patient’s experience of safe, timely discharge.
The post holder is directly responsible for the line management of the Discharge Co-ordinators and the provision of leadership and direction to these staff.
As part of a multi-disciplinary team within the Trust Discharge Team, provide discharge planning support where necessary and case management for the most complex group of patients.
Ensure patients and their families/carers are consulted and encouraged to be part of the discharge planning process.
Oversee the case management and co-ordination of the discharge plans for patients requiring complex support.
Ensure patients on a complex discharge plan are reviewed against their actual plan and take appropriate action when there is any delay.
Ensure smooth and timely communication between the Trust and partner agencies.
Support the Trust in managing the patient pathway and flow.
Ensure that all areas within the Trust are aware of key issues pertaining to discharge planning.
Act as a role model for all staff in the organisation.
Ensure the key performance indicators relating to discharge planning are met.
