Lead, deliver and supervise the delivery of expert wound care advice and support across SDHC, ensuring patients are receiving high quality, appropriate nursing care within all areas of responsibility working alongside all members of the MDT and students. Establish evidence based treatment plans, in partnership with patients and other professionals, using agreed protocols, but also where there are no appropriate precedents. Assess, diagnose, plan, implement and evaluate treatment plans for patients presenting with tissue viability needs and/or complex wounds, formulating and adjusting the plans where necessary. Triage and make referrals using expert judgement in defining the management of patients presenting with tissue viability and wound management needs across the hospital and community settings. Act as an expert practitioner demonstrating advance clinical competency and a knowledge base beyond that associated with traditional nursing such as Nurse Conservative Sharp Debridement. Act as an educational expert providing learning and support to all levels of clinical staff as well as patients and their families, forging links with local educational providers. Ensure that effective communication structures are in place to enable patients and relatives to understand the nature of the care, treatment Establish and maintain communication systems between referring consultants, ward staff, Integrated Locality Teams, GPs, practice nurses and other member of the MDT Maintain and develop documentation which accurately reflects the assessment, planning and evaluation of wound care. Identify and deal effectively with poor practice or declining standards of care and have an oversight of the resource management related to tissue viability products, ensuring responsible use of resources whilst promoting safe and effective care. Co-ordinate the tissue viability contribution to nursing practice policies & procedures across area of responsibility and trust wide. Professionally and legally responsible and accountable for own clinical and professional activities. Lead the implementation of the Trust clinical governance agenda within all areas of responsibility. Co-ordinate the investigation and resolution of clinical incidents within area of responsibility ensuring lessons learnt are implemented and audited. Represent the nursing views on tissue viability nursing at appropriate groups/ committees both internally and externally.and progress
