Job Description:
We are pleased to be able to offer a Full Time Band 5 Mental Health Practitioner post with the East Devon OPMH team based at St Johns Court in Exmouth.
The post holder will deliver high quality mental health services. The role involves managing a defined caseload. The focus will be providing continuous assessment and treatment to people with mental health presentations and their carers. This will include Key working (design and co-ordination of treatment packages that are personalised) and the liaison with a wide range of statutory and non-statutory agencies is central to this role. The post holder will be an effective member of the multi-disciplinary team.
Main duties of the job
* Contribute to the teams engagement including liaising with a wide range of other professionals and agencies.
* Impart complex and sensitive information to patients and families/carers regarding assessment, care planning, treatment, and review.
* Collaborate with multi-disciplinary teams to ensure the provision of consistent well-coordinated care.
* Liaise with primary care teams regarding client care.
* Engage with external agencies involved in the provision of care e.g.: voluntary and private sector, police, social services, housing, employment, etc.
* Contribute to the safe and effective day-to-day operations of the service including the handling of clinical inquiries.
* Provide and receive information, some of which may be contentious, sensitive, and complex, to and from individuals and their families/carers and actively seek support as appropriate.
* Provide high standards of written and verbal communication that is clear and in line with professional documentation standards.
* Participate in clinical meetings, reviews, professional meetings, and clinical risk meetings.
* Communicate information in a way that makes it relevant and understandable for service users and carers, working in line with practice standards and operational policies.

Job responsibilities
* Undertake risk assessments and determine the appropriate course of action, considering a range of possible options. Ensure that consultation with senior colleagues is undertaken where necessary.
* Undertake and continuously re-evaluate bio-psycho-social assessments and contribute to the development of a working formulation and diagnosis, in consultation with senior clinical staff and the multi-disciplinary team, leading to the development of personalised care packages in line with evidence-based practices.
* Maintain awareness of your own clinical competencies, knowledge base, and experience, ensuring that you practice within this and seek support as required.
* Ensure the continuous re-evaluation of individuals' needs and clinical risk situations and use clinical judgement to formulate the most appropriate clinical plan.
* Organise meetings, including professional and risk meetings.
* Manage your own workload to ensure the needs of the individuals who use our services are met, considering the wider needs of the team.
* Ensure that interventions are planned in an integrated and coordinated manner, which may include liaison with external agencies.

* Deliver care in line with evidence-based practice and employ engagement and therapeutic skills appropriate to the clinical situation presented.
* Provide specialist advice to referrers/other multi-disciplinary staff regarding the care of individuals.
* Enable communication and engagement of individuals and their carers to ensure they have a voice about the services they receive and how these are developed. This may include engagement of advocacy services.
* Ensure appropriate action is taken to safeguard adults and children, including attending safeguarding meetings and implementing relevant care plans under the guidance of a Senior Mental Health Practitioner.
* Ensure that assessment and treatment plans are holistic, outcome-focused, and consider the physical health and social needs of the individual, including input from significant others.
* Monitor and review mental health medication, considering interactions with physical health needs.
* Ensure timely and accurate documentation of all clinical activity in line with professional guidance and best practice.
* Practice in a way that actively minimises dependency and promotes recovery.
* Have a working knowledge of eligibility for services, including FAB/FACS assessments, and refer for these services when appropriate.
* Engage in the development of mental health services as guided by line management.
* Participate in the implementation of local systems and processes in your own area of work.
* Ensure compliance with CQC standards through participation in provider compliance assessment tools.
* Contribute to clinical governance arrangements to ensure the quality of service provided, ensuring performance targets are met to provide high-quality care.
* Prepare care panel applications as guided by the line manager or senior colleague.
* Ensure care is provided in the most cost-effective and high-quality manner, identifying unmet needs.

* An understanding of the NSF and Mental Health Act/Mental Health Capacity Act requirements.

* Experience managing a caseload of clients with a range of mental health presentations.

Qualifications
* An appropriate professional Mental Health qualification and registration e.g. RMN, OT, or Social Worker.
* Training to level 2 in Safeguarding Adults, children, and young people: roles and competencies for Health Care staff.
