Job summary An exciting opportunity has arisen to join our team of forward thinking Advanced Clinical Practitioners within acute medicine. The post holder will bebased at West Suffolk Foundation Trust (WSFT) on the Acute Assessment Unit and Same Day Emergency Care, within Acute Medicine. The Advanced Clinical Practitioner (ACP) will use a high degree of autonomy and complex decision making alongside demonstrating the four pillars of practice, Clinical Practice, Leadership and Management, Education and Research in providing healthcare that enables innovative solutions to enhance experience and improve outcomes within their speciality area. The ACP is accountable for independent clinical assessment, examination, diagnosis, and treatment within an agreed scope of practice.In addition to this the ACP is also responsible for appropriately referring patients to relevant specialities for any necessary inpatient or outpatient investigations as well as facilitating safe patient discharge. Main duties of the job The successful candidate will practice as an ACP using a high degree of autonomy and complex decision making to assess, investigate, diagnose, plan, implement, manage, and evaluate clinical nursing and medical care in the management of patients. You will be responsible for the initial treatment of patients presenting with undifferentiated or undiagnosed conditions and make professional autonomous decisions regarding treatment within your scope of practice. Draw upon a diverse range of knowledge to determine evidence based therapeutic interventions, which will include prescribing medication where legally allowed and review medications for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice, national and practice protocols, and within scope of practice to optimise treatment. Liaise with other members of the multidisciplinary team to allow the most appropriate treatment and plan the safe discharge of patients when required. Work in partnership with patients, families and carers and support the individual to proactively manage their wellbeing and life. Participate in audit, clinical governance and other clinical meetings where the outcomes and recommendations have the potential to improve the quality of care; give case presentations as required.
