We are looking for highly motivated Pharmacists to join our innovative Pharmacy Team to provide maternity cover (until approx. Spring 2026) for our lead Pharmacist, as well as a permanent position too. To start as soon as possible.
We are a large team consisting of Pharmacy Co-ordinators, Dispensers, a Pharmacy Technician and Pharmacy Apprentice, supporting a Practice population of around 28,000 patients. Working closely with our PCN Pharmacy Team, covering around 85,000 patients.
Main duties of the job
Work with the lead Pharmacist initially, supporting with leadership and management of the pharmacy team and cover whilst they are on maternity leave.
Provide quality clinical pharmacy expertise and be an educational resource for colleagues in relation to medicines, providing information re side effects, doses and alternatives, offering evidence-based advice in line with local and national guidelines.
Offer medication reviews (in person or via telephone), structured medication reviews for patients with complex polypharmacy and multi-morbidities.
Be an integral part of the long-term condition team and offer reviews of chronic diseases.
Support in achieving QoF targets, taking the lead for completion of QoF prescribing targets.
Lead the team in the annual Prescribing Quality Scheme (PQS) targets.
Support the team in all areas of medicines management and optimisation such as medicines reconciliation, completing audits, actioning drug and safety alerts, reviewing and producing new SOPs or policies when needed.
Promote efficient and safe repeat prescribing processes with a focus on patient safety, reducing overprescribing and medicines waste.
Have oversight of the onsite dispensary, with knowledge of how dispensing doctor sites work.
Support with Covid and flu vaccination campaigns.
Liaise with the PCN Pharmacy and wider team to ensure collaborative working.
Proactively develop relationships with the local ICB medicines management and Community Pharmacy teams.

Job responsibilities

- Be an integral part of the long-term condition (LTC) team and offer comprehensive and holistic reviews of chronic diseases such as hypertension, diabetes, COPD, asthma, lipid control, chronic kidney disease and heart failure.
- Support the practice in achieving QoF targets, specifically taking the lead for completion of QoF prescribing targets.
- Lead the Pharmacy Team in completing the annual Prescribing Quality Scheme (PQS) targets.
- Support colleagues in all areas of medicines management and optimisation such as medicines reconciliation, completing clinical audits, actioning drug and safety alerts, as well as reviewing and producing new SOPs or policies when needed.

Person Specification
Qualifications
* Completion of an undergraduate degree in Pharmacy and be registered with the General Pharmaceutical Council (GPhC).
* Minimum of 2 years postgraduate experience in pharmacy, with some experience working in general practice.
* Member of the Royal Pharmaceutical Society (RPS) (preferable).
* Hold an Independent prescribing qualification or be working towards this.
* Hold or be working towards a postgraduate pharmacy qualification.
* Experience in managing/leading a team (preferable).
* Have an interest and experience in long term condition/chronic disease management.
Experience
* Hold a Full Clean Driving Licence.
* Experience of using SystemOne is an advantage.
