South Warwickshire Foundation Trust Leg Ulcer Service, based in North Warwickshire, is pleased to offer an exciting opportunity for a Band 5 Staff Nurse for 30 hours per week.
The post holder needs to be a well-organised individual and have a genuine interest in the management of wound care. The exciting and rewarding role involves collaborating with the wider MDT (Vascular / Dermatology / Dieticians / Podiatry / Lymphoedema Services / District Nurses etc) to support individuals through their leg ulcer journey.
In this process, the individual will gain an array of transferrable skills and experience in developing individualised plans of care from evidence-based practice. Become proficient in undertaking the vascular assessment and interpretation of the findings and develop a secure knowledge base around wound care and dressing selection.
The Team is based at Camphill Health Centre and carry out clinic and home visits across North Warwickshire and Rugby. The service currently runs Mon - Fri 8:30 - 16:30 hrs.
The post is community-based, and the ability to meet the travel requirements of the role is essential. Applicants must hold a full driving license and have access to their own vehicle (car lease scheme in operation).
Main duties of the job
1. To support the day-to-day running of the nurse-led clinics in North Warwickshire.
2. To be responsible for the assessment and management of the health needs of an identified population and for the development, implementation, and evaluation of programmes of care for patients with lower leg ulceration.
3. To prescribe using the nurse formulary and to ensure that nurse prescribing is undertaken in a safe and cost-effective manner.
4. To provide support to the team including performance development reviews, clinical supervision, audit, education, and training related to leg ulcer management.
5. To assist in acting as co-ordinator in the care delivery of GP and District nurse caseloads.
6. To work across professional boundaries, providing comprehensive health care that is sensitive to the needs of the population.

Person Specification
Qualifications
* Registered Nurse with NMC (Adult Nursing)
* Evidence of continuous professional development
* Mentoring and/or mentoring qualification or similar
Experience

* Must be able to demonstrate up to date knowledge and wide range of clinical skills
