The post holder will work as an autonomous specialist nurse practitioner in the Rapid Access Chest Pain Clinic, using specialized skills and knowledge to assess, plan, implement, and evaluate care for patients to facilitate early diagnosis, investigation, and treatments.
Main duties of the job
1. Triage patient referrals to the Rapid Access Chest Pain service, received from GP or ED, and liaise with TAC to organize clinic appointments ensuring they are seen promptly within the National required time frame of 14 days.
2. Request relevant tests, prescribe medications if necessary, and make onward referrals.
3. Respond to test results and communicate these to the patient, GP, and other healthcare professionals.
4. Provide education and advice to ensure the patient is engaged with their treatment plan.
5. Work within the multidisciplinary team, with a weekly MDT with Consultant Cardiologist.
6. Participate in the development of policies, audits, clinical governance, and research.
