Mount Chambers has a fantastic opportunity for an experienced practice nurse to join our team on a part-time basis, for 25-30 hours per week on over 3 days. The main responsibilities of the role include: Undertaking health assessments, administering medication and treatments, and performing diagnostic tests. Providing and evaluating a range of health interventions to ensure that patients’ health and social care needs are met. General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition. Managing and treating long-term conditions, i n line with national and local policies and the practice needs. Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care. Supporting and managing the health needs of women presenting for sexual health and contraception, and cervical cytology consultations. Implementing, and participating in, vaccination and immunisation programmes for both adults and children, including patients travelling abroad. Meeting the needs of patients presenting for opportunistic wound care. Hypertension review and management. Running asthma clinics and reviews. New patient and health checks. Phlebotomy. About the Candidate All applicants should have the following qualifications, registrations, skills and experience: A relevant nursing degree. Current NMC registration with an active PIN. Fundamentals of Practice Nursing course (General Practice Nurse Foundation Programme) or equivalent experience or qualifications; Competence to undertake asthma, COPD and diabetic clinics and reviews, holding relevant certificates. Evidence of clinical skills and experience in treatment room consultation, phlebotomy, ECG recording, screening, management of long-term conditions, cervical cytology sampling, vaccinations and immunisations, assisting with minor surgery, dressing and wound care. Compassionate manner with an empathetic approach and demonstrable kindness in all aspects of your work. Knowledge, and experience of its application, in supporting long-term conditions. Insight and understanding of current issues in nursing and the wider NHS. About Us We are a friendly and supportive dispensing, training and research practice providing high quality care to the people of Braintree. We are based over two sites, and are a team of 4 GP partners, 3 salaried GPs, 3 nurse practitioners, 2 practice nurses and 3 HCAs.
