We are looking to appoint a number of qualified nurses to help us deliver our NHS CHC service across Birmingham and Solihull. The role is varied and interesting, involving assessing individuals in their own home or care settings. The assessment is a crucial stage of the process when considering if an individual is eligible for their care to be commissioned and funded by the NHS. We need nurses with the ability to holistically assess care needs, who are compassionate and caring with the skills to support individuals during what can be a difficult time.
Main duties of the job
We are a clinically led and supportive team working with some of the most vulnerable individuals in our community. There is an opportunity to plan care, review its effectiveness, and work with colleagues in the wider health and social care system to deliver the best outcomes for individuals found eligible for NHS CHC.
Main duties of the job:
1. To review any service and care commissioned regarding quality and appropriateness to the individual's need.
2. To ensure that the provider is delivering the requirements of the contract ensuring value for money in all matters relating to packages for NHS continuing healthcare.
3. The post holder will work jointly with health, social care, and independent sector colleagues to ensure care management processes are appropriate.
4. To follow the process laid out in the National Framework for NHS Continuing Healthcare; to include organisation, coordination, and participation in MDT eligibility recommendations to be verified by ICB.
5. Ensure assessments conducted are robust, transparent, and multi-agency (where required) for NHS Continuing Healthcare/NHS-funded Nursing Care.
6. The post holder will be responsible for a complex clinical caseload comprising: assessments, undertaking of reviews (3/12 CHC review), unscheduled reviews, Fast Tracks, and implementation of Personal Health Budgets along with ongoing Case Management of patients funded by Continuing Healthcare (CHC).

Detailed job description and main responsibilities:
To support the seamless implementation of the National Framework for Continuing Healthcare including contribution to the development of training programmes and delivery plans as required to facilitate others' learning.
Within Birmingham and Solihull ICB, provide advice, screen, and coordinate assessments against the National Framework for Continuing Healthcare and Funded Nursing Care within Nursing Homes.
To work with the Senior Nurses to process Funded Nursing Care and Continuing NHS Health Care Applications.
Influence best practice by providing professional nursing advice on a range of physical and mental health nursing issues.
Person specification
* Registered Nurse with a current NMC registration or equivalent.
* Previous experience of undertaking Continuing Care Assessments.
* Conversant with national and local guidance on NHS Continuing Healthcare/NHS Funded Care for Adults and with a detailed understanding of the correct use of the national tools (checklist, DST, Fast Track).
* Educated to degree level or equivalent level of experience working in a specialist area.
* Extensive nursing skills in Adults with complex healthcare needs.
* Strong verbal and written communication skills.
* Ability to analyse and interpret quantitative and qualitative data, using this to draw robust conclusions and recommendations.
* Ability to plan and implement internal policies, processes, and procedures.
* Knowledge of Commissioning arrangements.
* Master’s in healthcare-related area or equivalent level of knowledge and experience in relevant subject area.
* Evidence of having undertaken a leadership role in working across boundaries.
