We have 4 small friendly teams across Lincolnshire comprising of Occupational Therapists, Physiotherapists, Nurses, Pharmacy Technician, Promoting Independence Therapy Assistants and Promoting Independence Care Assistants. We are a growing team and looking for enthusiastic, friendly, and proactive therapists.
In our Pathway 1 Team, we follow the patient home and see them within 2 hours of their discharge. We listen to the patient of what matters most to them with regard to their personal and social circumstances, their physical, mental and emotional health and their carer's needs. This will include exercise programmes for their physical and mental wellbeing, recommending and providing the correct equipment, proactive work with patients to encourage independent living, educating patients on self-management of their health and wellbeing such understanding frailty symptoms and the importance of staying well physically and mentally for falls prevention, and so much more.
Main duties of the job
The Pathway 1 Discharge to Assess Service provides therapy-based rehabilitation and reablement to patients immediately following hospital discharge. It will be the role of the therapists to provide profession-specific assessments, create rehabilitation care plans and to work closely with the team of Advanced Therapy Assistant Practitioners, Promoting Independence Therapy Assistants and Promoting Independence Care Assistants to ensure patients are seen and reviewed in a timely manner to maximise recovery. To work closely with all members of the multi-disciplinary team and to work at times in conjunction with other agencies to determine a person's long-term care needs.

Essential
* Degree or equivalent diploma in Occupational Therapy or Physiotherapy
* Current Health and Care Professions Council (HCPC) Registration
* Basic IT skills
* Ability to successfully complete a 6 month preceptorship for newly qualified or return to practice physiotherapists and competency framework

* Member of RCOT or CSP
