As a Specialist Public Health Nurse, you will be a case holder for complex and vulnerable young people who are eligible for a Care Leavers Service. You will work as part of the district wide health team for Children in Care & Care Leavers.
The team works to promote strategies to improve health outcomes for Care Leavers. Enabling and empowering care leavers to be involved in the assessment of their health needs and the delivery of their health plan is a key element of the role.
You will work closely with Local Authority Fostering and Adoption Services, Children' & Families Trust, Health Visiting, School Nursing, Substance Misuse Services, Child & Adult Mental Health Practitioners and Allied Health Professionals on all aspects of the young person's health, wellbeing, and safeguarding.
The Specialist Public Health Nurse for Care Leavers is expected to display high personal levels of competence, judgment, integrity, and personal accountability. You will assist the team in providing a single point of contact for advice, support and information pertaining to the health needs of Care Leavers.
Main duties of the job
You will be required to:
1. Demonstrate the ability and willingness to foster trusting professional relationships with the young people to empower, enable and support them to make informed choices regarding their health status.
2. Engage with Care Leavers to ensure that their voice is heard and is utilised to influence and inform the shape and delivery of the service across the Bradford District.
3. Demonstrate a sound knowledge of the impact of trauma and ACES on young people and be proficient in providing a trauma informed approach to practice.
4. Promote understanding of the importance of their health and wellbeing, their attendance for health appointments and provide ongoing encouragement to enable them to utilise services to optimise their health and wellbeing.
5. Ensure that health interventions are underpinned by your understanding and competent use of the principles of advocacy, empowerment, facilitation, communication, co-ordination, and partnership both with individuals and families, and with professionals from health services and those from other statutory and non-statutory agencies.
6. As part of the multi-disciplinary team, develop Individual Health Plans, in consultation with young people, and in partnership with the young person's Social Worker or Personal Advisor, ensure the delivery of the actions of this plan, with a specific responsibility for coordinating the delivery of health services necessary, within agreed timescales and targets.
