Assessing nursing care and health education needs. Development, implementation and evaluation of programmes of care and pre-assessment clinics for patients undergoing a variety of treatments.
Designated to manage the ward and act as deputy for the ward manager. To develop the necessary skills and experience in ward management and provide leadership in the absence of the ward manager.
The role will provide a high standard of care to frailty patients, showing evidence of the specialist skills and knowledge that will enable the delivery of multidisciplinary and high standards of care for frail patients.
Supervising and teaching of registered and unregistered nursing staff alongside supporting supernumerary staff members. Work in collaboration with members of the multi-disciplinary team.
Co-ordinate the frailty unit which includes assessing Frailty patients in accident and emergency and supporting them in the transition through our ambulatory clinic and manage bed capacity on Bevan court 1 and 2.
Support and have knowledge of frailty syndromes and complexity of Frailty. Liaise with bed manager and attend bed meetings to provide update of flow within the unit.

Clinical & Professional Responsibilities
Maintain Professional Registration. Adhere to relevant Code of Practice of the Nursing and Midwifery Council. Adhere to the Trust policies and protocols.
To participate in the recruitment, orientation, development, and any other personal issues related to staff as required. Optimum use of resources so that standards of care and safety are not compromised.
Maintain effective communication systems, so that all staff can practice in an informed and professional manner. Maintain effective communication with patients, relatives, visitors, and staff as required.
To investigate any untoward incident/complaint thoroughly. Liaise promptly with the Ward Manager, Matron or Directorate Manager. Deputise for Senior Nursing staff as directed.
Be fully conversant with the Major Incident Policy and be prepared to support its activation as required. Ensure effective infection prevention and control methods are followed.
Maintain effective allocation of staff utilising appropriate skill mix.
Administrative Responsibilities
To ensure that the recording of patient care is comprehensive, concise and in line with the NMC Record Keeping guidance. Ensure IT systems are current eg ADT to ensure that all patient information is confidential and is not disclosed to a third party except with permission of the patient or the Manager in Charge.
To keep patient records up to date on a day-to-day basis following any interaction with patients/relatives. To ensure all unit documentation is completed.
TEACHING & TRAINING RESPONSIBILITIES
To maintain an effective learning environment for all staff, contributing to the training and assessment of staff. To act as a role model and expert practitioner, providing support and guidance to nursing staff.
To participate in the annual performance review of identified staff Band 5 and below in order to identify their training and development needs. To participate in an annual performance review to identify the post-holders personal development needs.
To personally ensure fulfilment of the requirements to maintain active and valid professional registration, personal, and professional development and NMC revalidation. To assist with induction, preceptorship, and mentorship programmes for new staff as appropriate.
Line Management/Supervisory Responsibilities
Responsibility for designated nursing staff within team including appraisals, HR management and support in clinical practice in conjunction with the Ward Manager. Responsibility for nursing staff within the team in the absence of ward manager or deputy ward manager.
To regularly take charge of the clinical area in the absence of a senior member of staff. Participate in meetings pertinent to the Unit.
To be responsible for ensuring all patients and their relatives/carers are kept fully informed of patients treatment/progress and discharge plans/after care. To report all accidents, hazards, incidents, and complaints in accordance with Trust policies and assist in investigation of them as required.
Comply with Health and Safety policies.
FINANCIAL RESPONSIBILITY
Maintain efficiency and minimise any waste. Managing nursing team and skill mix appropriately, utilising bank and agency staff when appropriate within ward budget.
Provide information to enable income and expenditure from activity to be recorded accurately and in a timely manner.
RESEARCH & AUDIT
Responsible for following clinical trials protocols. Involvement in potential audit or research within the unit or Trust.
