Our Values: Job Summary: Working as part of a Multi-Disciplinary Team (MDT), the job holder will play a key role in the development of Primary Care Networks in delivering enhanced care at GP practice level. As a Primary Care Paramedic, the job holder’s primary role will be to work autonomously throughout the Hillingdon area, to provide first point of contact for patients presenting with undiagnosed problems relating to minor illness or injury using their existing enhanced clinical assessment and treatment skills. The Primary Care Paramedic function will support PCN GPs by making urgent visits to patients, initiate appropriate care and effective treatment to patients in a pre-hospital environment, selecting and applying appropriate skills and equipment safely and (within the scope of their role) support the progression of care plans of patients with long term medical conditions, working to improve the health and wellbeing of the people of Hillingdon. The PCN Paramedic will also be given dedicated time to help develop services and clinics for the PCN, playing a key role in how the PCN functions going forward, strong involvement in teaching and taking on a leadership or managerial role where appropriate. Primary Responsibilities Assess and triage patients, including same day triage, and as appropriate provide definitive treatment or make necessary referrals to other members of the PCN team Required to use their own car for transport/ patient home visiting. Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to other community or voluntary services. Perform specialist health checks and reviews. Perform and interpret ECGs; alongside other results as appropriate.Perform investigatory procedures as required. Undertake the collection of pathological specimens including intravenous blood samples, swabs, etc. Perform investigatory procedures needed by patients and those requested by GPs and the wider PCN. S upport the delivery of anticipatory care plans and lead certain community services (e.g. monitoring blood pressure and diabetes risk of elderly patients living in sheltered housing). Provide an alternative model to urgent and same day home visits for the PCN and undertake clinical audits. Communicate at all levels across PCNs and other organizations, ensuring effective, patient-centred service. Communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required. Collaborate with other members of the PCN including doctors, nurses and other AHPs, accepting referrals and referring to them for specialist care. Maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice. Prescribe, issue and review medications as appropriate following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways. This will be done in accordance with evidence-based practice and national and practice protocols, and within scope of practice. Work with patients in order to support compliance with and adherence to prescribed treatments. Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions. Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis, and patients with complex needs. Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care. Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan. Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care. Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures. Be aware of data protection (GDPR) and confidentiality issues particularly within a PCN. Use technology and appropriate software as an aid to management in planning, implementation and monitoring of care, presenting and communicating information. Review and process data using accurate read codes in order to ensure easy and accurate information retrieval for monitoring and audit processes. Supervision: The post holder will have access to appropriate clinical supervision and an appropriate named individual in the PCN to provide general advice and support on a day to day basis. There will also be a Clinical Lead at the Confederation level providing support for teaching, training and peer support. Person Specification: Essential Criteria: BSc in a training programme approved by the College of Paramedics. Health & Care Professions Council (HCPC) registration. Able to operate at an advanced level of clinical practice. Framework for Higher Education Qualification (FHEQ) Level 7 or Scottish Credit and Qualifications Framework (SCOF) Level 11. Has completed their two-year ‘Consolidation of Learning’ period as a “newly qualified paramedic” Experience in managing a designated caseload using theoretical and practical experience, completing assessments, planning, implementing interventions, and evaluating outcomes aligned to care plans. Working knowledge of Microsoft and GP practice and prescribing data monitoring systems. Ability to write comprehensive clinical notes, implement and evaluate care plans. Working towards advanced clinical practitioner status. Ability to evidence a sound understanding of the NHS principles and values. Desirable Criteria: Pre-reg MSc in a training programme approved by the College of Paramedics Non-medical prescribing qualification and has a further three years’ experience as a band 6 (or equivalent) paramedic Mentorship or supervisory skills training and ability to undertake nails surgery assessments, procedure and post-operative care. Be aware of data protection (GDPR) and confidentiality issues particularly within a PCN. Knowledge or experiences of using EMIS GP system. Cognitive behavioural and motivational interviewing approaches / skills. Experience of working within a primary care setting. Evidence of working across organisational boundaries within health and social care.
