An exciting opportunity has arisen to join the District Nursing Service for a Community Staff Nurse covering the Out of hours service in North Tyneside and Northumberland.
We are looking for enthusiastic staff nurses to join our district nursing teams based at The Health and Care Academy. This is a fantastic opportunity to become a team member in an exciting service committed to providing patient centred care to patients in their own homes, residential care settings and in clinics. We are keen to provide our staff with development opportunities to enhance their skills, knowledge and career progression within the District Nursing Service, while providing high quality and safe service in line with the Trust Values.
Successful applicants must hold a full UK driving licence and have access to a vehicle.

Responsibilities:
1. Work autonomously and as part of a team, reporting directly to the community nursing team lead.
2. Assess, plan, implement and evaluate care for a diverse group of patients with a wide range of conditions and health needs.
3. Develop and deliver programmes of health and social care.
4. Provide care and support to patients with a terminal illness and their families, including emotional and bereavement support.
5. Perform skilled nursing care including aspects of assessment, wound management, palliative care and catheterisation.
6. Conduct diagnostic tests such as Ankle Brachial Pressure Index using Doppler and interpret results for further action.
7. Perform procedures requiring fine dexterity such as venepuncture, drug administration by injection, and removal of sutures.
8. Interpret results and communicate these to patients, allowing opportunities for questions and concerns.
9. Maintain high standards of professional practice, sharing approaches and experiences with others.
10. Maintain a duty of care to patients, carers and colleagues through personal and professional accountability.
