An exciting opportunity has arisen for a Care Coordinator with 3 years' experience to join a well-established home care provider. This full-time role offers excellent benefits and a salary range of £24,000 - £25,000.
As a Care Coordinator, you will coordinate care services, ensuring the smooth operation of client rosters and providing assessments for new and existing clients.
Responsibilities:
1. Ensure care is provided in line with relevant policies and regulations.
2. Conduct client assessments to determine individual care needs.
3. Perform regular reassessments of current care plans.
4. Liaise with families, medical professionals, and other stakeholders.
5. Carry out spot checks, supervisions, and appraisals for care staff.
6. Efficiently manage staff rotas, ensuring compliance with hours and breaks.
7. Ensure staff recruitment files are up-to-date following interviews.
What we are looking for:
1. Previously worked as a Care Coordinator, Care Supervisor, Care Worker, Support Worker, Care Assistant or in a similar role.
2. At least 3 years' experience in providing care.
3. Knowledge of the Bucks area.
4. Valid driving licence.
5. Right to work in the UK.
