Due to expansion, The Integrated Complex Assessment Team have a vacancy for a Complex Assessment Nurse.
The integrated Complex Assessment team is responsible for the assessment and tracking of patients within a (D2A) discharge to assess bed based service within a timely manner relieving the pressures to the acute hospitals.
The role involves working closely with multi-disciplinary teams, service users and relatives performing early review of service users transferred into assessment beds to establish needs and funding requirements for discharge.
Following the national framework for continuing Healthcare and NHS funded Care, the post holder will be responsible for the completion and sharing of timely assessments and funding applications.
The Post holder should be able to prioritise their own workload to ensure effectiveness and efficiency in delivering objectives whilst working with minimal supervision. Being proactive and able to demonstrate good organisational skills and excellent communication skills both in verbal and written correspondence.
The role is varied and requires confidentiality, initiative, flexibility and capability of working under pressure and to deadlines.
The Post holder must be able to travel over the D2A footprint and have excellent written and verbal communication skills.
Main duties of the job
* The assessments of those with complex needs across all discharge to assess beds for the organisation.
* Care planning and assessment supporting the MDT to establish best outcomes.
* Managing own workload.
* Performing early review (meet and greet) of all those transferred to assessment beds to establish needs and goals.
* Working closely with external providers, individuals and their families to ensure best outcomes for all.
* Palliative assessment and care planning.
* Completing assessments and funding applications utilising appropriate NHS frameworks.

* To support the Team in the provision of a high quality service to individuals and their families, who require health assessments, rehabilitation following acute hospital admission.
* Provide advice and support to junior staff and learners in all aspects of the delivery of high quality care.
* Work closely in partnership with other multi-disciplinary team members to ensure effective care planning and delivery.
* Service hours are Monday to Friday.
* The Nurse assessor will provide support, assistance and professional expertise to both professionals and patients.
* To deliver expert knowledge and assessment skills to support the MDT in delivering a comprehensive and holistic assessment.
* To pro-actively case manage complex patient discharge assessments from admission to discharge.
* To complete assessments in line with DOH guidelines and recommendations, having expert knowledge of all continuing health care assessments and the fast track assessment process.
* Responsible for completing initial assessments to guide the MDT in delivering holistic and timely assessments.
* Leading multidisciplinary team meetings.
* Supporting flow into and out of the assessment beds.
* Communicating effectively with patients, families, independent providers and the multidisciplinary team.
* To be able to work independently and within a multidisciplinary team.
* To evaluate patient progress and provide guidance and advice in regards to identifying optimum assessment opportunity.
* Undertake risk assessments to support the discharges of patients with complex needs.
* Consider consent and capacity and have an understanding of the health and social care act.
