Job summary We have exciting opportunities for you to join our All Age Complex & Continuing Care (AACCC) team, working across Buckinghamshire, Oxfordshire and Berkshire West. We are looking for highly motivated Registered Nurses of all disciplines AND/OR Allied Health Professionals who can work autonomously and collaboratively in a multi-agency environment to support the assessment and review process for NHS Continuing Health Care (CHC) and NHS Funded Nursing Care (FNC). Prior knowledge of AACCC would be beneficial but is not essential as you will work within an experienced and supportive team who will support you as you settle into your new role. You will be involved in patient assessment and reviews, care planning and case management as well as audit, appeals, retrospective assessments, complaints, assessment verification, and much more. Support will be available from colleagues and senior management to help you develop the necessary skills and experience to work across the whole pathway, safely, confidently and effectively. At the same time, as you demonstrate your expertise and skills you will have autonomy to plan and prioritise your own workload to meet the requirements of the post. There is the opportunity to work full time - or part time - on a hybrid basis and in addition we support flexible working options to enhance the work/life balance of our teams. We operate within core business hours, Monday to Friday, excluding bank holidays. Main duties of the job The post holder will be responsible for undertaking specialist health needs assessments for individuals who are undergoing assessment for, or who are already eligible for, NHS Continuing Healthcare and NHS Funded Nursing Care. Assessments are generally carried out within the community in residential settings but may also take a hybrid approach. Therefore, you will need to have access to a car to support you with your role. Travel expenses will be reimbursed in line with NHS Policy. You will be required to work with the individual and/or their representative, and to liaise with private providers, partner organisations, along with other health and social care professionals to ensure that assessments are person centred.
