Meridian Business Support are working on behalf of a Further Education College based in Greater Manchester. They are looking for an experienced SEND Transition Facilitator to provide support to their students.
Role: SEND Transition Facilitator
Location: Manchester
Pay rate: £20 - £25 per hour
Start date: Monday 17th March 2025
Duration: 1 month
Role responsibilities:
1. Works to support the work environment to adapt job requirements to student skills.
2. Monitor and problem solve students' performance for progress monitoring.
3. Maintain clear, accurate records of student performance and communicate with students, staff, administrators, and employers.
4. Maintain employer standards and build positive, long-term relationships with employers.
5. Provide transportation training and assist students in accessing transportation.
6. Assist students to acquire independent living skills in the classroom and community.
7. Support integration of assistive technology for students.
8. Occasionally provide physical assistance to students with special daily living needs.
9. Identify potential students who may need transport and support them with the application process, ensuring that timetables are supplied to the transport team.
10. Liaise with parents, carers and potential students to ensure that an individual transition plan is in place where required.
11. Attend school EHCP reviews as required.
12. Identify current students who may need support to transition to their next steps, both within college and externally, and work with the SEND team to ensure this is in place.
13. Arrange and host visits to the college for SEND students, in conjunction with the School Liaison team and campus SEND teams.
Hours of work: 34 hours per week, Monday - Friday
Ideal candidates will have:
1. Experience working with students in a mentoring or SEND facilitator role.
2. DBS on the update service.
3. Experience supporting students within secondary or a college.
4. Experience working with students with disabilities.
