Castlefields Health Centre is looking to expand its nursing team by employing an additional experienced part-time Practice Nurse, 20 hours per week. We are a busy General Practice situated in the heart of the Castlefields Estate in Runcorn, Cheshire. The successful candidate will join an existing team of Practice Nurses, so you will have plenty of support! Your day-to-day role will see you managing your own clinics as well as carrying out the occasional home visits.
You will be an experienced nurse, who is responsible for ensuring the delivery of safe and effective patient care to the practice population using autonomous clinical skills and a broad in-depth theoretical knowledge base.
You will manage a diverse clinical case load, dealing with the presenting patients' needs within a primary care setting. You will contribute to developing the nursing services offered by the practice. You will work independently and contribute to audits, research and supporting, implementing and evaluating local standards, policies, guidelines and care pathways.
NMC requirements will be met and you will work within the NMC Code of Conduct to agreed practice standards and protocols.
Main duties of the job
You will see a variety of patients in your clinics and will carry out the following:
1. Make professionally autonomous decisions for which you are accountable for providing safe, evidence-based, cost-effective, individualised patient care.
2. Assess, plan, develop, implement and evaluate treatment programmes that promote health and well-being.
3. Specialist primary nurse-led services, including managing programmes of care for patients with chronic diseases (including for all areas of QOF).
4. Where necessary evaluate patient test results in conjunction with the patient and their GP and ensure the appropriate course of treatment is initiated.
5. Administer child and adult immunisations and vaccinations in accordance with national and local programmes.
6. Cytology screening and promotion of breast screening/bowel screening (where appropriate).
7. Be competent in anaphylaxis and resuscitation techniques.
8. Promote health and well-being, giving general education advice on diet, contraception, smoking, chronic disease management, exercise etc.
9. Initiate and carry out programmes of health screening as agreed with the practice clinical team.
10. Maintain accurate records according to NMC rules.
11. Work within the multi-disciplinary team within the practice and across the wider health system.
12. Maintain and keep up to date with mandatory and job role specific training.

We are a training Practice with 3 of our GP Partners being GP trainers. 2 of our GP Partners were once GP trainees at Castlefields Health Centre!

Job responsibilities
1. Undertake a range of nursing assessments and provide appropriate care and treatment in conjunction with the GPs, Nurse Practitioners and other practice nurses or independently, in line with competence and need.
2. Maintain or improve standards of care and be actively involved in identifying practice population needs.
3. To demonstrate and apply knowledge related to the Quality and Outcomes Framework (QOF) and provide therapeutic interventions within a broader health promotion/public health strategy and enable patients to take action to meet their priorities for health.
4. Provide education and training regarding chronic clinical conditions as required.
5. Undertake any further training required to ensure appropriate role development.
6. Practice within the NMC Code of Professional Conduct and maintain accurate records.
7. Assist practice GPs/NP to deliver effective and holistic management of patients with chronic diseases as required by the Quality and Outcomes framework.
8. Contribute to the development of policies / protocols / guidelines within the practice.
9. Aid the practice team with implementation of new initiatives in the delivery of chronic disease management/enhanced services.
10. Support the implementation of evidence-based guidelines in conjunction with the practice clinical team.
11. Promote the continuing professional development and skills of both yourself and other nurses within the practice.
12. Participate in chronic disease management within the practice & provide practical assistance and professional advice to achieve the highest standards of care.
13. Provide clinical input where required alongside other practice nurses, to assist in skill development and sharing of best practice.
14. Maintain nursing practice in line with relevant evidence-based research.
15. Act as a resource to the general public, patients within the health care system, their families, carers and external healthcare professionals.
16. Work alongside other practice staff to help develop their knowledge and skills.
17. Support the practice in aiming to achieve 100% QOF and enhanced service targets.
18. Undertake opportunistic screening when clinically required/indicated.
19. Perform venepuncture and electrocardiography where required.
20. Perform cervical screening and promote breast awareness.
21. Administer adult and paediatric injections under an individualised prescription or Patient Group Direction, ensuring safe storage, rotation and disposal of vaccines and drugs.
22. Undertake accredited training/updates in chronic disease management.
23. Contribute to the maintenance of the practice disease registers.
24. Provide advice on lifestyle issues, assist and support patients in changing their behaviours and refer appropriately.
25. Support the adult and childhood immunisation service working to Patient Group Directions.
26. Deliver travel healthcare, including vaccinations and advice for patients.
27. Recognise the signs of child abuse, family violence, vulnerable adults, substance abuse, addictive behaviours, suicidal tendencies and demonstrate awareness of local referral procedures.
28. Apply infection control measures when collecting and handling laboratory specimens, disposing of waste materials, dealing with blood and body fluid spillages and report and treat sharps injuries according to protocol.
29. Undertake annual training and demonstrate competency in anaphylaxis and resuscitation techniques Research, Audit and Quality.
30. Contribute to the provision of equitable, high-quality services for patients in the locality.
31. Maintain an interest in national research initiatives (NICE) and disseminate appropriate information to health professionals as required.
32. Ensure all care is evidence-based using up-to-date validated literature.
33. Provide professional advice/support to healthcare professionals within the Primary health care team.
34. Participate in partnership programmes, raising awareness, detection and prevention of chronic diseases.
35. Maintain a high level of expertise in specific chronic disease management, specifically Diabetes, COPD, Hypertension, CKD, CHD, AF, & Asthma and seek to develop new and improved skills.
36. Advise members of the public on the concepts of health promotion, education, prevention and protection.
37. Seek out new knowledge of nursing and health by reading, enquiring and partaking in internal and external continuous education programmes.
38. Take responsibility for personal development and education including in-house training & monthly PLT.
39. Actively participate in multidisciplinary education and training.
40. Monitor and maintain consistency of professional standards and take appropriate action if standards are not met.
41. Maintain professional development by attending relevant courses and study days to keep abreast of current knowledge and developments.
42. Abide by the legal requirements and statutory rules relating to practice and maintain periodic professional registration with the NMC.
