Job summary We are seeking a motivated and enthusiastic pharmacist to work in our innovative and supportive Renal Medicines Service. You will care for people with kidney disease across South West Wales from our integrated multi-disciplinary hub in Morriston Hospital. Increasing demand, exciting new medication and a welcome focus on early identification and effective holistic management of CKD in primary and secondary care is an opportunity for pharmacy to lead improvements in the health and wellbeing of our population. You will provide expert advice to clinicians and inpatients with chronic and acute kidney disease, people receiving dialysis and kidney transplant recipients. Specialist areas include post-kidney transplant care, renal anaemia, mineral and bone disorder, and vasculitis. This role will also develop your knowledge and understanding of cardiovascular disease, Acute Kidney Injury, peri-operative pharmacological management, antimicrobial stewardship, and symptom control. You will prescribe regularly (with the required qualifications). You will work across multiple interfaces, including primary to secondary care, inter-Health Board and inter-speciality, and with commissioners, independent service providers, academia and the third sector. You will lead exciting audit, quality improvement and research projects. You will contribute to the education and training of our multi-professional colleagues, and the long-term development of the Renal Medicines Service. Main duties of the job Provide pharmacological advice and care for people with kidney disease on the renal ward in Morriston Hospital, and to outpatients across the South West Wales renal region Review drug charts Contribute to the renal pharmacy dispensing service Effectively manage and prioritise workload Interpret appropriate evidence, and local and national guidelines in order to formulate pharmaceutical care plans Actively contribute to multidisciplinary and departmental meetings Effectively analyse and interpret laboratory data Operate as an independent prescriber (with relevant qualifications and experience) Undertake clinical risk management within all areas of pharmaceutical care Ensure compliance with pharmacy protocols and procedures Identify problems with patient understanding or ability to take medication and provide suitable solutions Ensure adherence to relevant protocols, guidelines, and formulary Ensure the effective storage and stock control of pharmaceuticals in areas ofresponsibility Supervision of pharmacy technicians and assistants, and clerical support staff Contribute to and implement drug related policies and guidelines Actively undertake audit and quality improvement (QI) projects Provide education and training to colleagues Monitor renal drug use and expenditure, and contribute to relevant reports Deal with difficult situations effectively Contribute to the digital development and sustainability of the renal medicines service About us We believe staff are our best asset and wewant you to be happy and confident about starting your career here in Swansea Bay University Health Board.
