Job overview
An exciting opportunity has arisen for a dynamic occupational therapist to join the Community Therapy team in Bassetlaw. The team provides short-term rehabilitation to older people in their own homes to avoid admission to hospital and support discharges from hospital (D2A).
This is a challenging but rewarding role with a strong multidisciplinary approach within the team.
The post covers the Bassetlaw area and is based at Retford PCC, working shifts between 08:00 and 20:00 seven days a week to meet the diverse needs of this patient group.
In addition to support from your team leader and the MDT, OT specific support is provided by monthly peer supervision and forums which offer support and CPD.
This is a 9-month maternity leave cover for 30 hours per week.
Main duties of the job
1. To provide an occupational therapy service for older people with complex health needs who are referred to the service.
2. To ensure a high-quality Occupational Therapy Service is provided through multi-disciplinary, multi-agency working.
3. To carry out specialist assessment, goal setting, implementation of treatment programmes and evaluation.
4. To establish the needs of individuals referred to the service and plan care with colleagues from health, social services and the voluntary sector.
5. To represent and promote the specialist role of Occupational Therapy within the Community Team including involvement with service planning and development.
6. To provide specialist advice, rehabilitation and equipment to the population of Bassetlaw who access the service.

Detailed job description and main responsibilities
The post holder must have a BSc (Hons) in Occupational Therapy or equivalent and current HCPC registration. Experience of working in the community and within a team are essential. Also essential are a positive attitude to older people with health and social care needs.
A full UK driving licence and vehicle for business use is required for this post; however, reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
Clinical Practice and Work Practice
1. To be professionally and legally responsible for all aspects of own work, ensuring a high standard of clinical care for the patients/clients under your management.
2. To demonstrate ability to carry out specialist assessment, treatment planning and goal-oriented intervention for a designated caseload.
3. To demonstrate the ability to select and use appropriate therapeutic interventions with patients/clients/carers, including group interventions.
4. To ensure effective discharge planning and facilitate seamless transitions of care.
5. To assess, supply and monitor equipment to maximise patient/client function.
