Jacksonville, FL 32257Under the direction of the Corporate Medical Records Manager, the Health Information Specialist 2 supports and strengthens the organization Medical Records/Health Information Management Department through analysis and maintenance of patient medical records, releases of medical records, death registration and supporting other processes as required. The HIM Specialist 2 will serve as a liaison between patients, providers, health care facilities, insurance companies, and other establishments in regards to health information.
Confidentiality:
1. Adheres to the requirements of the HIPAA Privacy Policies and Procedures. Maintains confidentiality of health information of patients, families, and staff.

Records Maintenance & EMR:
3. Maintains assigned medical records according to federal/state regulations and organizational policy;
4. Ensures patient records are complete, accurate, and available for care, treatment, and payment at all time within the EMR and document management system;
5. Researches and corrects errors on patient records and demographics in the EMR (Electronic Medical Record) as needed;
6. Performs audits of the medical records to ensure compliance with departmental and regulatory requirements;
Records Analysis & Deficiency Monitoring:
7. Audits and analyses all patient records to ensure compliance with regulatory requirements (i.e. admission, discharge, orders, etc…;
8. Identifies trends, patterns, and opportunities for improvement by notifying providers and other staff of any deficiencies in the record and tracks responses to ensure timely correction.

10. Pulls, compiles, and analyzes reports to increase efficiency and allow for real-time access to patient information.
Release of information:
11. Receives, retrieves, and releases medical records after reviews to outside healthcare providers, third party payers, government/state agencies, patients, insurances, and/or legal parties in accordance with applicable policies and guidelines;
12. Trains new employees and/or interns in the release of information process;
13. Completes first level review of records retrieved prior to releases and completes and process invoices for records.
14. Navigates multiple EMRs, document management systems, and various software applications to fulfill requests.
Scanning/Imaging:
15. Scans and/or indexes patient/client record into the electronic medical record (EMR); Performs quality control audits and correct errors in scanned or paper documents.
Registry Maintenance:
16. Initiates and completes electronic death certificates via state electronic death registration system (EDRS) and according to state and Community Hospice guidelines;
17. Works as a liaison between physicians, funeral homes directors, medical examiner’s office as well as the department of Health bureau of Vital Statistics to ensure timely completion of the death certificates within the state guidelines;
18. Completes amendment procedure and facilitates notarization process...
19. Trains new employees in the Death registry process.
20. Assists the HIM Senior Specialist in enrolling new physicians in the death registry process.
Records Destruction:
21. Assists in the destruction of paper records according to CHPC record retention and destruction guidelines.
22. Maintains the destruction log as needed.
Mail and Correspondence:
23. Fulfills all correspondence request in a timely manner;
24. Receives and distributes incoming departmental mail daily;
25. Collects and distributes outgoing mail daily.
26. Keeps manager informed of issues or concerns as they arise. Performs other duties as may be required to support the organization’s medical records program.

Education/Experience:
Any combination of education and experience that would provide required skills and knowledge for successful performance would be qualifying.
Required Qualifications:
28. High School diploma or Equivalent required
29. Experience: 2-4 years in a medical records/HIM setting and at least 1 year of healthcare experience preferred.
30. One (1) year computer/data entry experience.
31. Certified Electronic Health Record Specialist (CEHRS)is preferred or willing to obtain within 90 days.
32. Associate’s degree in Health Information Management (HIM) preferred
33. Ability to pass AHCA Level 2 background check.
Knowledge of:
34. Numeric filing.
35. Other Electronic Medical Record (EMR) systems.
36. Microsoft Office applications: Word, Excel, Office, PowerPoint, Adobe Pdf, cloud and web based functionality.
37. Basic HIPAA regulations.
