An exciting opportunity has arisen for a Support Worker to join our Cancer team at Luton & Dunstable Hospital. This post offers an exciting opportunity to help shape cancer care pathways, patient support and management. The role is designed to strengthen existing supportive care services by providing support to the operational delivery and co-ordination of care for patients.
We are looking for a highly motivated individual who can communicate effectively and confidently whilst working intrinsically with MDT teams and colleagues within the acute setting and across the local community, both independently and as part of a team.
Main duties of the job
The post holder will be the key person identified for providing cancer information and support, to enable people to navigate the health and social care system and make choices that are best for managing their cancer journey. The aim of the role is to make sure that people living with cancer can access the right support, at the right time.
The post holder will take an active role in contributing towards the development and implementation of cancer survivorship initiatives enabling people affected by cancer to navigate the health and social care system and become self-supportive, i.e., co-ordinating health and wellbeing clinics to enable independence and support self-management as appropriate.
The post holder will co-ordinate care by providing a single point of access for cancer patients including rapid re-entry.
Job responsibilities
* Provide regular supported conversations with the person living with cancer, assessing their holistic needs, and supporting them to self-manage by providing relevant information and advice, and signposting to other Macmillan resources and services available to meet their identified needs.
* Following a holistic needs assessment, co-ordinate care and development of a personalised care plan with the person living with cancer so they can access the right help at the right time and providing a personalised response to any changes in need.
* Provides practical and emotional support to people living with cancer and their families. Recognising information needs may change over time and working with the person to ensure they have all the information and support they need at all stages of their cancer journey.
* Assess and interpret the needs of individuals attending/contacting the service and respond effectively to the needs of individuals providing a professional quality service.
* Triage individuals contacting the service face to face or by telephone/email and work autonomously to prioritise needs and initiate appropriate responses according to protocols and individual pathways.
Person Specification
Qualifications
* GCSE English Language and Mathematics (Grades A-C) or equivalent.
* Foundation degree level training or equivalent.
* NVQ4 or NVQ3 plus additional training to diploma level equivalent, or equivalent through short courses.
* Evidence of continued role development.
* Willingness to undertake Macmillan learning and development courses.
* Coaching or teaching qualification.
Experience
* Relevant health or social care experience at AFC Band 3 or equivalent.
* Experience of multi-professional working.
* Experience in the use of data management.
* Evidence of good communications skills.
* Understanding of person-centred care.
* Understanding of the health and social care environment.
* Experience in coordinating a patient workload.
* Awareness of Improving Outcomes: A Strategy for Cancer and other national cancer policies.
* Experience of coaching/teaching patients and carers.
