Band 7 All Age Continuing Care Team Leader

An exciting opportunity has arisen for an appropriately skilled and experienced Registered Nurse/Social Worker/Allied Health Professional to join NHS Somerset's All Age Continuing Healthcare (CHC) team as a Team Leader.
The post holder will contribute clinical expertise to the NHS continuing healthcare (CHC) and Funded Nursing Care (FNC) process. They will lead a team to deliver multi-disciplinary assessments and reviews in a nursing home or community setting for patients both within Somerset and out of county in line with standard operating policy and procedures.
The vacancy is for 37.5 hours a week but part-time hours can be considered.

Main duties of the job
* To manage the day-to-day service requirement of CHC, FNC, and Personal Health Budgets (PHBs).
* Lead and manage CHC Assessors, including supervision, appraisal, and performance review.
* Coordinate reviews for existing CHC patients and ensure consistency in applying CHC eligibility policies.
* Lead CHC assessments and reviews, including out-of-county cases and complex situations requiring expert management.
* Plan and coordinate complex care packages and Personal Health Budgets.
* Promote compliance with the 28-day timeframe from checklist to decision.
* Build effective relationships across NHS and non-NHS organisations at local and national levels.
* Support organisational development in personal health budget pathways and the personalization agenda.
* Oversee individual care contracts, ensuring quality and safety in partnership with commissioning managers.
* Ensure excellent customer care throughout the CHC process, resolving concerns locally where possible.
* Prepare appeal cases and liaise with families in sensitive or contentious situations.
* Foster patient and carer involvement throughout the CHC process.
* Collaborate with local authorities on CHC decision-making and outcomes.
* Serve as an expert resource for health and social care professionals, patients, and families.
* Investigate and address CHC complaints alongside the Operational Manager.

PURPOSE OF THE ROLE:
The post holder will be responsible for:
Contributing clinical expertise to the continuing healthcare process, leading a team to deliver multi-disciplinary assessments and reviews for patients including those in Somerset and out of county in line with standard operating policy and procedures.
KEY RESPONSIBILITIES OF THE ROLE:
* To manage the day-to-day service requirement of the CHC, FNC and PHBs.
* To directly manage and provide expert leadership to a team of CHC Assessors including all managerial supervision, appraisal, professional leadership, performance review and management as needed.
* To provide line management cover in the absence of Team Leaders to Assessment Teams as needed.
* To coordinate reviews for existing CHC patients.
* To ensure consistency in the application of national and local policy on eligibility for CHC.
* To identify and support any requirement in relation to performance of staff and work with HR as appropriate, which may include formal review of capability.
* To lead on CHC assessments and reviews for patients in Somerset and provide care management for CHC patients placed out of county.
* To provide assessment of highly complex cases which require expert management due to difficult circumstances.
* To coordinate highly complex packages of care to meet individuals' needs.
* To coordinate and plan Personal Health Budgets for eligible patients.
* To maintain full awareness of ethical considerations with regard to CHC issues.
* To actively promote and drive compliance with the 28-day timeframe from checklist to decision.
* To undertake regular quality assurance reviews of new applications to support quality and consistency.
* To provide expertise to CHC panels and to the wider CHC team in relation to CHC applications and care provision.

* To support the development across the organisation in the personalisation agenda and to support the pathway delivery for Personal Health Budgets within NHS Continuing Healthcare.
* To coordinate and monitor individual contracts for packages of care.
* To work with Contract and Commissioning Managers to ensure that Providers of care to CHC funded patients are delivering care that is safe and effective through clinical quality contract review.
* To undertake and support audit requirements relating to CHC.
* To support the development of and implement local policies, systems and processes around CHC, linked to the relevant key national and local priorities.
* To promote excellent customer care throughout the CHC process ensuring local resolution of any concerns where possible.
* To support service requirements, undertake training and assessments (Best Interest Assessor) relating to Deprivation of Liberty Safeguards (DoLS).
* To ensure effective timely communication and liaison with key stakeholders, including partner agencies, GPs, patients and their families, and care providers.
* To work with CHC manager and cascade information as appropriate.
* To be responsible for providing first level contact for disputes between partner agencies or with patients/families/legal bodies.
* To undertake preparation of appeals cases within the defined policy, including the up-to-date clinical information required and involving liaison with families in complex situations.
* To implement patient/carer involvement at all stages of the CHC process.
* To ensure that the local authority is involved appropriately in the CHC decision-making process.
* To promote awareness of NHS funded CHC.
* To provide information relating to the monitoring of CHC activity and contract compliance.
* To act as an expert resource to health and social care workers, patients, families and other organisations and individuals.
* To investigate and respond to any complaints about CHC in partnership with the CHC Operational Manager.
