In this role you will be the lead Pharmacist for the provision of comprehensive clinical pharmacy services to the Virtual Wards as well as supporting our primary care network. You will be embedded in the multidisciplinary team providing care to respiratory, cardiology and frailty patients in the Virtual Wards. In addition, you will support further development of clinical and pharmaceutical pathways. We are looking for someone who has extensive clinical pharmacy knowledge and experience, and the ability to work with a multidisciplinary team. You will need to have excellent communication, clinical and negotiation skills, as well as experience of supervising other staff and providing education and training. Main duties of the job Responsible for the provision and further development of clinical pharmacy services across Shropshire, Telford and Wrekin (STW). In conjunction with the principal pharmacist, attend and actively participate in Medicine Governance meetings and to work collaboratively with the Medicines Management Team to support delivery of the departmental vision and associated strategic objectives. Be a key member of the multidisciplinary team in relation to virtual ward. Support development of the virtual ward which provides an alternative clinical pathway for respiratory and frailty patients and those with long term conditions. Advise the Principal Pharmacist of any new developments within the Community Services to ensure that the Pharmacy continues to evolve and meets the changing requirements of clinical governance and medicines optimisation.
