ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Perform comprehensive patient assessments to determine functional deficit and potential and establish a therapeutic program. Develop a therapy plan of care to include goals and discharge plans.
2. Document, within time frames, observations, patient's response to treatment, treatment programs, degree of progress and notable changes in patient condition. Completes all documentation required and in accordance with state and federal regulations and policy. Demonstrate administration of objective, standardized tests and measures and performance of evidence-based practice in documentation.
3. Communicate and coordinate patient care with other members of the patient's health care team. Attend employee meetings and patient conferences.
4. Supervise physical therapy assistants in accordance with regulations and policy.
5. Provide patient and family teaching related to the patient's physical therapy program and patient/family engagement in goal achievement. Assist the patient and/or family to procure needed equipment or supplies.
POSITION REQUIREMENTS:
* Graduate of an accredited Physical Therapy program
* Licensed as a PT in Massachusetts
* Home care experience strongly preferred.
* Current CPR required.
* Current State of Residence driver's license and reliable transportation
