Job summary We are recruiting an experienced Practice Nurse to work 24-27 hours per week across 3 days and join our multi-disciplinary clinical team providing a high standard of care for our practice population of 4,500 patients. Our team are friendly, approachable and support each other well ensuring a pleasant working environment. We are in a modern purpose built building, have free staff parking and are close to good public transport links. Main duties of the job The practice nurse role involves following practice processes and national guidance for the effective management of patients with long-term conditions using evidence-based practice including care for elderly and housebound patients. Working as a senior member of the practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively to the patient population. The practice nurse will be responsible for a number of clinical areas such as infection prevention and control, health promotion, chronic disease management, health prevention, as well as actively supporting the practice management team in the reviewing and delivery of clinical policy and procedure. About us Our clinical team is led by 2 GP partners and comprises of GP's, ACP's, Practice Nurse, Healthcare Assistants and Pharmacist. These are all supported by our reception team.
