HBTT is a true alternative to hospital admission, supporting people to remain in their own homes while receiving a high level of care and treatment for their mental health.
We work within both medical and social models of care and use a strengths-based approach to recovery, ensuring service users receive the right care at the right time. We work closely with community teams across the South Manchester footprint to ensure consistent and knowledgeable care.
Our team is truly multidisciplinary—our Mental Health Practitioners are from various professional backgrounds: RMNs, Social Workers, Occupational Therapists. We work alongside our medical team and Clinical Psychologist with a recovery focus.
We believe that staff well-being is the building block of a great team and ensure that staff are valued and supported through regular supervision, team formulations, regular team training, and development opportunities.
Your role within the team will allow you to develop excellent skills in both bio-psycho-social assessment and risk management, alongside key working service users who require full HBTT support.
Main duties of the job
An exciting opportunity has arisen for a Band 6 Mental Health Practitioner to join the Home Based Treatment Team in South Manchester. We welcome practitioners from a nursing, social work, or occupational therapy discipline.
This is a fantastic opportunity to join an enthusiastic and dynamic community team. Home Based Treatment Team provides an alternative to inpatient admission and supports people with severe and acute mental health problems in their home while undergoing intensive treatment for their mental health needs.
Candidates will be expected to work as a keyworker providing assessments and informing treatment plans for those people under Home Based Treatment. You will be a good communicator and self-motivator who delivers high-quality care. You will be expected to carry out in-depth assessments in the community and contribute to and formulate care plans and risk management plans.
Home Based Treatment Team is a 24/7 service, and you will be working on a rota basis, including some weekends, evenings, and nights when the service requires.

Person specification
* Must have professional registration: NMC, Social Work England, HCPC
* Post-qualifying training in psycho-social interventions
Experience
* Relevant experience in urgent care and/or community
* Experience of managing and formulating risk and completing assessments
Knowledge
* Excellent risk assessment and management skills
* Experience of suicide prevention, completing MSE
Skills and abilities
* Good assessment skills
* Ability to work alone
* Confident independent decision-making, experience of assessing patients independently
