Main duties of the job
The post holder will be assisting in the provision of a safe, efficient and comprehensive pharmacological service for the patients of Medway Community Healthcare CIC.
They will assist the head of medicines management at MCH in the provision of professional/clinical advice, delivering medicines-related training, undertaking medication reviews, audits, and research.
The role includes opportunities to develop innovative patient-centred medicine optimisation projects across the community services whilst working with the pharmacy team at Medway NHS Foundation Trust.
The post holder will liaise between both clinical leads at MCH and MFT to ensure that pharmacy services are tailored to meet the Trust's and patients' needs.

Job responsibilities
To provide and develop a comprehensive, patient-focused, clinical pharmacy service to different specialties promoting the safe, appropriate, and clinically effective use of drug therapy.
To frequently communicate complex drug or medicine-related information to patients and carers, tailoring the information to the patients' specific needs. The patients may have language difficulties, physical, or mental difficulties.
Provide patient-focused pharmaceutical care to patients, to achieve definite outcomes that improve patients' quality of life.
To maximise patient care on defined areas within the Trust by planning, managing, monitoring, advising, and reviewing specialist pharmaceutical care programmes for patients.
Evaluate the clinical appropriateness, safety, and legality of prescriptions based upon the patients' diseases, co-morbidities, other medication, and treatments and liaise with the prescriber to make agreed changes as appropriate.
Person Specification
Qualifications
* Vocational Master's degree in Pharmacy and successful completion of pre-registration training and experience
* Registered with the General Pharmaceutical Council as a Pharmacist in the UK (or eligible to be)
* Postgraduate Diploma or equivalent (or be working towards one)
Experience
* Hospital pharmacy experience
* Understanding of working effectively within multidisciplinary groups
Knowledge
* Good core therapeutic knowledge including expert knowledge in medicines legislation and guidelines
Skills
* Evidence of excellent communication skills (verbal and written) with healthcare professionals and patients
* Methodical and able to prioritise workload
