An exciting opportunity to join the DTV ALD Physical Health Team as a Nursing Associate.
The post holder will work as part of the Physical Health Teams, working to break down barriers to improve health outcomes and quality of life for people with a learning disability. The team covers Durham Tees Valley, and the appointed applicant will be based within the Durham area.
The Health Facilitation Team is currently merging with the Teesside Enhancing Physical Health Team to become the Durham Tees Valley Physical Health Team.
The service operates from 9am to 5pm, Monday to Friday, and the post holder will also be part of the Nurse on Call rota that covers out of hours from 5pm to 9am.
Main Duties of the Job
Applicants must have NMC Nursing Associate registration and preferably experience working with people with a diagnosed learning disability.
The successful candidate will work with multi-professional practitioners to ensure collaborative working with service users, their families, carers, and partner agencies, providing a person-centred approach.
Excellent communication skills, calmness, confidence, and resilience are essential, along with the ability to form therapeutic relationships.
A flexible approach to work is necessary, as participation in the on-call rota is expected, and the ability to travel independently to various locations across County Durham is required.

Person Specification
Qualifications
* Registered Nursing Associate on the NMC register.
* GCSE Grade A-C in Maths & English or skills level 2 Maths & English or equivalent.
* Qualification in assessment, supervision, and teaching in clinical practice.
Knowledge/Skills
* Experience of working in teams under appropriate supervision as part of a multi-disciplinary team.
* Experience of providing and receiving complex, sensitive information.
* Ability to organise and prioritise own delegated workload.
* Ability to communicate effectively (written, verbal, and non-verbal) with patients/relatives, carers, and all members of the multi-disciplinary team.
Experience
* Up-to-date understanding of the scope of and role of the Nursing Associate in the context of the nursing and multidisciplinary team and how the role may contribute to service development.
* Understands and acts in line with NMC professional standards for practice contained within The Code.
* Experience in working within learning disability/mental health services.
* Knowledge and experience of safeguarding, assessing risk, and documentation in line with TEWV policies.
Other Requirements
* Ability to travel independently in accordance with Trust policies and service needs.
