An opportunity has arisen for a motivated and compassionate band 7 Urology Clinical Nurse Specialist. We are looking for a dynamic, experienced nurse with good communication skills, who has an interest in developing their CNS skills or extending their current portfolio of skills and abilities. You need to be able to prioritise your workload and assist us in meeting NHS cancer waiting times and personalised cancer care targets, whilst maintaining the patient experience and supporting them and their families through their pathway.
You will ideally be an experienced urology nurse, having had the experience of providing personalised care to people with prostate cancer in the urology and oncology setting. You should have excellent time management and advanced communication skills.
Experience in running nurse led clinics would be desirable although not essential.

Main Duties of the Job
The post holder will work as a key member of the Urology multi-disciplinary team.
1. To act as a resource of expert advice, carry own case load and provide coordination of care and support to patients with Urology malignancies and their family / carer from diagnosis and through all stages of their disease, treatment modalities and follow up care.
2. To liaise and work closely with other professionals involved in the patient's care journey and provide specialist knowledge, expertise and education for staff within the Trust to ensure a smooth patient pathway for all patients with Urology malignancies.
3. To develop and monitor standards of care for patients following National guidelines.
4. To act as a key worker for patients with or are suspected of having Urological cancer, ensuring patients have information details of the MDT and contact number of key worker.
5. To ensure all National data requirements are collected & entered on the appropriate databases, including local and national audits such as NPCA & Quality of Care.

Job Responsibilities
Professional / Clinical
1. To communicate and work in partnership with all members of the MDTs to ensure a seamless pathway of care for patients from diagnosis through treatment and follow up care.
2. To be an autonomous practitioner and manage own caseload and work with colleagues to provide the highest possible standards of care for patients diagnosed with a urology cancer.
3. To support and co-ordinate patients / family / carer from diagnosis onwards through all stages of their disease and treatment modalities and utilise excellent communication skills to complete appropriate onward referrals for your cohort of patients.
4. To act as point of contact and offer to be the Urology keyworker for patients and their families and carer.
5. Ensure the Holistic Nurse Assessments are completed for all patients at key points of their cancer treatment pathway. You will be required to liaise with and ensure referral and appropriate signposting onto allied health professionals and services as required.
6. To provide and offer a wide range of information and support to patients their family and/or carers at all stages of their cancer journey.
7. To competently act as the patient's advocate and assist patients in understanding their disease and treatments available to them, to enable them to make informed choices about their care.
8. Assist in the co-ordination of patient treatment pathways between the various departments in both primary and secondary care to facilitate continuity of care.
9. To attend the relevant Urology outpatient clinics.
10. To be a core member of the Urology MDT and attend the palliative care and site-specific MDTs where Urology patients are being discussed and as deemed appropriate.
11. To establish links and develop networking opportunities with other Urology CNSs.
12. To support and assess all urology cancer in-patients and liaise with ward staff and Consultants as deemed appropriate.
13. To ensure that patients are referred onto other health care professionals and services to ensure a seamless, high-quality pathway of care for them, their families and carers.
14. To establish and maintain close working relationships with the Hospital and relevant Community Macmillan Palliative care teams.
15. To be responsible in ensuring accurate and comprehensive documentation in all patients' notes.
16. To work as part of the MDTs core member and actively participate in the National Cancer Peer review process.
17. To regularly attend the Cancer Clinical Nurse Specialist Forum and participate in the agreed projects/work streams.
18. To provide CNS cover for cancer/Urology CNS colleagues during leave and absence.
19. To be competent in managing your own timetable and caseload.
20. To ensure that robust PSA patient tracking is completed in partnership with the MDT Coordinator.
21. To possess exemplary time management skills.
Education and Training
1. In conjunction with the Lead Cancer Nurse, identify education and training needs of relevant staff and develop education programmes to meet the identified needs.
2. Ensure learning materials are available in clinical areas.
3. Continually update and maintain own personal development and specialist knowledge base.
4. To be responsible for undertaking all statutory and mandatory training updates.
5. Act as specialist resource and provide information and advice for staff.
Quality, Research and Audit
1. Participate in both clinical and organisational audits.
2. To attend, participate and present at departmental audit meetings.
3. To undertake research/audit topics appropriate to Urology and disseminate findings.
Management and Supervision Responsibilities
1. To constantly review the urology cancers service provision.
2. To undertake and understand appropriate risk within area of practice.
3. Act as a resource within your role as a senior nurse.
Person Specification
Education and Qualifications Essential
* First level RGN registration
* Teaching qualification or extensive experience
* Educated to degree level or currently pursuing a degree
* Relevant post-registration qualification in urology care, oncology, or palliative care
* Evidence of recent professional development
* Experience in urology and/or oncology at Band 6 level or above
Skills and Abilities Essential
* Proven ability to teach and educate nurses, allied health professionals (AHPs), and patients
* Excellent written, verbal, and presentation skills
* Highly developed interpersonal skills, with the ability to inspire and motivate teams

Knowledge and Experience Essential
