Local Care Direct is a non-profit Social Enterprise operating across 24 hours a day, 7 days a week, 365 days a year, to provide various primary and urgent health care services across West Yorkshire.
We have various part-time positions available for Qualified Advanced Clinical Practitioners at Wharfedale Urgent Treatment Centre in Otley, Leeds. These are also opportunities to develop as we are keen to support aspiring colleagues into leadership roles.
Wharfedale Urgent Treatment Centre is an alternative to A&E, treating both children and adults. The centre provides treatment for minor injuries and illnesses that are urgent but not life or limb threatening.
Hourly Rate : £ in hours & £ out of hours
(In Hours = Monday to Friday to & Out of Hours = Monday to Friday to and all day Saturday Sunday)

Main duties of the job
The Advanced Clinical Practitioner will practice, without direct supervision, as an independent primary / urgent care healthcare practitioner within various clinical environments, participating in care of all age ranges and presenting conditions.
The Advanced Clinical Practitioner will undertake assessment, diagnosis, treatment / referral and discharge of patients with primary / urgent care needs within their clinical competence.
The Advanced Clinical Practitioner will provide clinical mentorship and leadership to all trainee and peer practitioners within the workplace when required.

Job Purpose:

KEY PRINCIPLES
Clinical
22. Independently prioritise, assess, treat, and safely discharge/refer patients attending with undifferentiated and undiagnosed urgent primary health care problems
23. Undertake a range of clinical consultations via a range of modalities primarily face to face but may include telephone and home visits (service specific).
24. Act as a referral agent and care co-ordinator by establishing multi-agency collaboration across the primary/secondary interface to meet the needs of the patient.
25. Work within the parameters of current prescribing legislation or approved Patient Group Directives.
26. Provide professional and clinical advice and direction to colleagues within your team.
27. Maintain accurate, contemporaneous records.
Research and Quality
28. Demonstrate evidence-based care, working within agreed guidelines and protocols.
29. Participate in audit, data collection and activity monitoring to improve performance and inform future service developments.
30. Proactivity report patient safety incidents in line with NHSs Just Culture and respond to Local Care Directs Quality Teams requests for information during incident investigations.
31. Take an active role in the management of safety incidents and organisational learning.

32. Actively promote integrated working in collaboration with other health care professionals both within and outside the LCD organisation.
33. Contribute to the development, implementation, review and evaluation of clinical protocols.
34. Ensure effective communication, participating in meetings and liaising with other professionals, services and agencies as required.

35. Develop own skills and competencies through education, training and development activities related to the current and future demands of the role, seeking opportunities to enhance skills, undertake annual internal performance development review (PDR), in order to maintain current Registration and achieve Revalidation standards.

39. Participate in teaching and mentoring other health professionals including, but not exclusive of, trainee ACP, trainee UCP and GP Registrars from Deanery.

To ensure agreed lines of communication and reporting are implemented within the team to support all employees understanding LCD business plans, objectives, priorities and changes.
To promote effective cross-functional working and that the operational team supports the achievement of objectives, standards and protocols of other LCD areas.
To attend internal and external meetings representing LCD and engaging in discussions appropriate to the role and level of the post.

TRAINING
To complete the defined company induction programme and core training for this role and mandatory refresher training thereafter and ensure that accurate records of training are maintained.
INFECTION PREVENTION AND CONTROL

40. Evidence of a range of clinical expertise, working independently in a healthcare role.
41. Experience of working in an autonomous practitioner role. Evidence of competence in the clinical assessment, diagnosis and management of adults and paediatrics presenting with undifferentiated diagnosis.
42. Experience of managing minor illness.

Qualifications

47. One of the following: NMC Registered General Nurse OR BSc in Paramedic Science / IHCD Paramedic qualification OR BSc in Physiotherapy OR MPharm AND Advanced Clinical Practitioner MSc/Post Grad Diploma in advanced clinical practice (minimum 1 year post Masters study).
48. Independent Prescribing qualification.
49. Current registration with HCPC, NMC or GPhC.
