Job Description:
The Case Manager, Youth Worker (Tailored Learning) is responsible for the coordination of the timely and effective case management service in accordance with the departmental Case Management Service Standards. The case manager supports at-risk, disengaged students in their social and emotional wellbeing, encouraging their continued connection with schooling through provision of case management services and access to accredited Tailored Learning. The case manager ensures appropriate options and interventions are identified and utilised to provide positive experiences for students in the wider community that result in the overall enhancement of their wellbeing and successful engagement with learning.
* Coordinate tailored learning and wellbeing programs, provide individual and group support, and make appropriate referrals to address social, emotional, and educational needs.
* Work with educators, external providers, and community agencies to plan and deliver personalized pathways, including SACE/VET programs, while respecting cultural diversity.
* Facilitate school and community connections to encourage ongoing student engagement, support at-risk transitions, and enhance wellbeing through positive community experiences.
* Maintain confidential case records, monitor attendance, complete progress reports, and share relevant information with staff and stakeholders as needed.
* Adhere to policies, maintain workplace health and safety standards, and ensure compliance with WHS and equal opportunity legislation.
Selection Criteria:
* Effective Communication and Collaboration: Proven ability to communicate clearly, maintain confidentiality, and build positive relationships with diverse stakeholders, including leaders, staff, students, families, and community members.
* Support for Vulnerable Students: Experience working with complex, at-risk youth and their families to drive positive change through mediation, conflict resolution, and tailored support strategies.
* Independent Problem-Solving: Demonstrated ability to work autonomously, using judgment, initiative, and problem-solving skills in high-pressure environments.
* Case Management Expertise: Skilled in providing case management services to young people facing mental health, social, or wellbeing barriers, improving their engagement, participation, and academic outcomes.
* Knowledge and Compliance: Comprehensive understanding of effective case management strategies, socio-economic diversity, and legislative requirements, including WHS and Equal Opportunity laws.
