Job summary An exciting 12 month opportunity has arisen for a band 4 Community Stroke Rehabilitation Assistant to work within the Knowsley Community Stroke Rehabilitation service. You will be an integral part of the Knowsley Community CVD Service. Instrumental in assisting the Community Stroke Team to delivery patient rehabilitation programmes to patients registered with Knowsley GP's who have had a CVA. You will provide appropriate support and rehabilitation within the community setting working across a variety of venues and within patients homes. You will work across the disciplines of Physiotherapy, Occupational Therapy, Speech and Language Therapy to development, coordination and delivery a comprehensive programme of Stroke Rehabilitation.
