We are now looking to recruit a Clinical Nurse Specialist Hospital2Home to work as part of the multi-professional team. In the first instance, you will need to be a registered nurse with at least 3 years post-registration experience in cancer/palliative care, and community experience would be a bonus. Educated to at least degree level, you will also need to be confident in delivering education and training to all staff groups, having attained some form of post-registration qualification in this. This post will offer this service to patients at both the Chelsea and Sutton sites of the hospital. We would welcome and encourage an informal visit.
This role is fixed term until October 2025 to cover maternity leave.
Main duties of the job
The post holder will be expected to liaise appropriately with all key members of the multi-disciplinary team and service users related to the specialty. In addition, the post holder will be required to liaise with their peers across the Trust, the divisional management team, and appropriate corporate directorates and teams. The post holder will be expected to liaise with colleagues external to the Trust and national networks as appropriate to the specialty.

Job responsibilities
* To identify appropriate patients in the outpatient setting and also those who may be occasionally referred from the hospital setting and to assess their community needs in order to facilitate a seamless and safe discharge into the community.
* To communicate directly with members of the community MDT including GPs, community palliative care teams, district nurses, and any other health care professional involved directly in the patients' discharge.
* To arrange telephone/face-to-face conferences with community colleagues and to participate in these meetings either by telephone or in the patients' preferred place of care.
* Along with the Matron/Nurse Consultant Palliative Care, support educational initiatives for community and primary care providers to improve knowledge and to empower local teams to provide supportive and palliative care services to patients in their own homes.
* To work with patients and their families to ensure a smooth transition between primary, secondary, and tertiary care services.
