Manorand Park Group Practice is looking for a motivated Pharmacy Technicians with astrong desire for professional development to join an established clinicalpharmacy team of 2 clinical pharmacists and 7 GPs.
ThePrimary Care Pharmacy Technician post is suitable for registered PharmacyTechnicians already working in primary care.
Thesuccessful applicants will work alongside the existing clinical team and besupervised and mentored by a named dedicated and experienced clinicalpharmacist.

Main duties of the job

Thepost holder will work within their clinical competencies as part of amulti-disciplinary team to provide expertise in clinical medicines management,management of medicines on transfer of care, manage repeat prescriptionauthorisations and reauthorisation and acute prescription requests, whileaddressing both the public health and social care needs of patients registeredat the Surgery.
Pharmacytechnicians core role responsibilities will cover clinical, and technical andadministrative categories.
Thepurpose of the role is to lead improvements to maximise safe, cost-effectivebest practice in prescribing to improve the quality of patient care. The postholder will help patients to get the best from their medicines by switchingmedications to agreed and approved protocols, improving repeat prescribingprocesses in General Practice, including promotion of repeat dispensing andonline ordering, minimising clinical risk and aiming to reduce wastedmedicines.
Inaddition, the post holder will be responsible for encouraging the developmentof better understanding of the principles of medicines optimisation throughoutthe practice teams and promoting good practice in line with therapeuticdevelopments. This will involve assisting the Surgery in achieving nationalrequirements, NICE implementation and utilisation of medicines optimisationinitiatives.

Job Description
The Post Holder will undertakepatient facing and patient supporting roles to ensure effective medicines use,through shared decision-making conversations with patients.
Carryout medicines optimisation tasks including effective medicine administration.( checking inhaler technique, supporting medication reviews and medicinesreconciliation.
Whererequired, utilise consultation skills to work in partnership with patients toensure they use their medicines effectively.
Asdetermined by the Surgery, support medication reviews and medicinesreconciliation for new care home patients and synchronising medicines forpatient transfers between care settings, linking with local communitypharmacies
Supportthe Clinical Pharmacist in Structure Medication Reviews undertakemedicines use review and organise necessary monitoring tests prior to SMR
Provideexpertise to address both the public health and social care needs of patients,including lifestyle advice, service information, and help in tackling localhealth inequalities.
Manageshared care protocols and liaise with Clinical Pharmacists for more complexpatients.
Supportinitiatives for antimicrobial stewardship to reduce inappropriate antibioticprescribing locally.
Technical and Administrative Responsibilities
Supportthe Surgery multidisciplinary team to ensure efficient medicines optimisationprocesses are being followed.
Implementefficient ordering and return processes and reducing medication wastage.
Providetraining and support on the legal, safe and secure handling of medicines,including the implementation of the Electronic Prescription Service.
Promotionof Electronic Repeat Dispensing and online ordering
Developrelationships with other pharmacy professionals and members of themultidisciplinary team to support integration across health and social careincluding primary care, community pharmacy, secondary care and mental health.
Supportpractice reception teams in streaming general prescription requests, to allowGPs and clinical pharmacists to review the more clinically complex requests.
Supportthe implementation of national prescribing policies and guidance within GPpractices, care homes and other primary care settings.
Supportthe Surgery to deliver on QIPP agenda, IIF, QOF and locally commissionedenhanced services
Supportthe Surgery in reviewing and developingpractice policies for CQC requirements.
