The purpose of the role is to oversee the quality of care within our new 12 Flat Scheme Supported Living service in the Lancaster area. The position also requires an ongoing focus on customer satisfaction and the consistent monitoring of care provision against customer need, and where necessary taking action to rectify problems. Responsibilities Delivery of Care · Support the Scheme in the delivery of care including delivering hands on care · Deliver and audit logbooks and MAR charts monthly for clients within the scheme · Report to the Scheme Manager or Care Coordinator any changes to care needs or medication, as required. · Support the office and on-call in the day-to-day resolution of care delivery issues. · Attend joint review meetings with multi-disciplinary agencies as required. Supervision of Care Workers · Undertake spot checks and medication competency checks where required. · Supervise Care and Support Workers day to day to ensure excellent delivery of service · Participate in new Care & Support Workers induction (including shadow training and mentoring). · Help to identify Care and Support Worker training requirements in accordance with customer need and regulatory requirements. · To organise and participate in regular team meetings within your own area with direction and support from the management team Customer Care · Complete all required documentation accurately and in accordance with company policy and procedure · Report any concerns or issues relating to the welfare of the customer or equipment to the Care Co-ordinator or Scheme Manager. · Attend all mandatory training events in accordance with regulatory requirements and Company policy · Attend supervision, appraisal, and team meetings in accordance with regulatory requirement and company policy · At all times present a professional and caring image of the branch, the division, and the Company and demonstrate behaviours reflective of Bare Hall Quality Carers values. · Build and maintain constructive relationships with customers, relatives, colleagues and health and social care professionals. Housing /scheme · Be aware of the tenancy agreement and the do’s and don’ts · Support the clients to maintain tenancy agreement · Organise tenant meeting including everyone, including using tech if needed. · Support clients with reporting and housing repair needs to the housing provider / landlord · Ensure that communal areas are clear of hazards. · Support Scheme manager to do weekly checks of the building · Support Scheme manager to ensure repairs are completed in a timely manner.
