We are looking for a compassionate and dedicated Community Support Worker to join our team. In this role, you will provide assistance and support to individuals in the community who may need help with daily living, social integration, and accessing services. Your goal is to enhance the well-being, independence, and quality of life of the people you work with, by offering emotional, practical, and social support in various settings. Key Responsibilities: Direct Client Support: Provide one-on-one support to individuals facing physical, emotional, or mental health challenges. Assist clients in their daily activities, including personal care, meal preparation, transportation, and social interaction. Help clients develop life skills, cope with challenges, and achieve personal goals. Community Engagement: Foster positive relationships with community resources, agencies, and organisations to ensure clients can access available support and services. Record Keeping and Reporting: Maintain accurate and up-to-date records of client progress, challenges, and goals through daily notes. Adhere to confidentiality and privacy regulations. Collaboration with Team: Work closely with a multidisciplinary team, including social workers, healthcare providers, other support workers and family members to coordinate care and services. Required Qualifications: Experience: Previous experience in a care-giving, social services, or community support role is essential. Skills: Strong interpersonal and communication skills, with the ability to work well with diverse populations. Empathy, patience, and the ability to build trusting relationships with clients. Problem-solving and crisis intervention skills. Ability to work independently and as part of a team. Proficient in record-keeping and using basic computer programs (e.g., Microsoft Office). Common sense Preferred Qualifications: Experience working with individuals facing mental health challenges and disabilities Knowledge of community resources and services. Fluency in English is compulsory. Working Conditions: The role may involve working in a variety of community settings, including clients' homes, college, community centres or other public spaces. Flexible hours may be required, including evenings and weekends. Occasional travel within the community.
